;. TUBERCULOSE LARÍNGEA DURANTE TRATAMENTO COM ADALIMUMABE NA DOENÇA DE CROHN. ABCDExpress. 2017;1(2):1120.Instituição: UNESP Apresentação do caso: homem, 44 anos, em monoterapia com infliximabe desde 2004. Teste tuberculínico (PPD) não reator e radiografia de tórax (raio x) normal pré biológico. Em 2016, em remissão clínica e endoscópica, decidiu-se pela troca por adalimumabe (ADA) pela comodidade da aplicação. Passados 2 meses, iniciou-se quadro de perda ponderal, febre, constipação e provas inflamatórias aumentadas. Não havia queixas respiratórias e raio x de tórax era normal. Ileocolonoscopia e trânsito de delgado mostravam doença em remissão. Associou-se odinofagia importante, que somada à hiperemia e hipertrofia amigdalianas discretas, foi interpretada a princípio como de origem infecciosa não micobacteriana. Instituídos antibióticos, com melhora parcial apenas após Levofloxacina. Prosseguida então investigação de neoplasia com nasolaringoscopia, que evidenciou lesão descrita como semelhante à estomatite moriforme (sugestiva de Paracoccidioidomicose), da prega ariepiglótica direita até a valécula, seguida de tomografia com lesão em lobo superior de pulmão direito que poderia corresponder a processo infeccioso de disseminação endobrônquica. Escarro negativo para fungos e bacilos álcool ácido resistentes (BAAR), mas biópsias das lesões pulmonar e da laringe com granulomas e pesquisa de BAAR positiva. Suspenso ADA, tratada tuberculose (com resolução dos sintomas) e programada reintrodução do imunobiológico adiante. Discussão: Estima-se que a infecção ativa pelo Mycobacterium tuberculosis entre os pacientes tratados com agentes anti fator de necrose tumoral ? (anti-TNF ?) seja de 2 a 20 vezes maior que na população geral. Na maioria das vezes, a infecção se dá por reativação de tuberculose pulmonar latente. Por isso, preconiza-se o rastreio antes do anti-TNF ? através de estratificação epidemiológica de risco, PPD e raio x de tórax. Entretanto, o PPD pode estar negativo inclusive em infecções ativas e o raio x de tórax pode não mostrar alterações sutis. Dentre as lesões granulomatosas da laringe, a tuberculose é a causa mais comum e pode apresentar-se isolada ou associada ao acometimento pulmonar, o que é mais frequente (até 37%, contra 19% da forma laringofaríngea isolada). É indispensável a nasolaringoscopia com biópsia e pesquisa de BAAR para o diagnóstico. Comentários finais: a despeito de testes de rastreio negativos e apresentações clínicas atípicas, a tuberculose deve ser uma entidade sempre considerada e insistentemente pesquisada em pacientes tratados com anti-TNF ?.
