We report a case of acute coronary syndrome with transient prominent anterior QRS forces (PAF) caused by proximal subocclusion of the left anterior descending (LAD) coronary artery before the first septal perforator branch. The ECG change indicates left septal fascicular block (LSFB) with associated slurring-type giant J-wave. Currently, this J-wave variant is considered as a lambda-like wave or QRS-ST-T "triangulation". Its presence is indicative of poor prognosis because of the risk for cardiac arrest as a consequence of ventricular tachycardia/ventricular fibrillation (VT/VF).
O curso clínico da cardiomiopatia hipertrófica evoluiu consideravelmente nos últimos anos, baseado em novas linhas de tratamento. O nosso objetivo foi realizar uma revisão de literatura focada nas características clínicas, diagnóstico e tratamento desta doença na população adulta. A utilização do eletrocardiograma além do exame clínico mostrou-se essencial para o diagnóstico, e o ecocardiograma torna-se imprescindível para o diagnóstico e acompanhamento dos pacientes. Em relação ao tratamento, observou-se que existem medidas não farmacológicas e farmacológicas; entre os não farmacológicos, a restrição de atividades físicas continua a ser a orientação comportamental mais eficaz, enquanto o tratamento farmacológico envolve principalmente o uso de betabloqueadores como primeira linha, tendo como outras alternativas os antiarrítmicos e os bloqueadores dos canais de cálcio. Além disso, há tratamento invasivo para a cardiomiopatia hipertrófica, sendo restrito a pacientes com alta gravidade ou refratários ao tratamento clínico, com sintomas limitantes.
