Based on general toxicological knowledge and the notorious measurements of corrosion parameters this paper is an attempt to define the biocompatibility of metallic materials used for implantation in the human body. Mentioned are essentially those implant materials and its metallic elements which are subjectmatter of a standard specification. The previous experiences concerning "metal allergy" are shortly recapitulated. Model conceptions about the mechanisms of tissue reaction to metallic elements are indicated.

Die Steigerung der Biegewechseldauerfestigkeit in Laborluft ist mit ca. 50 % nahezu unabhängig vom Strahlmittel, insbesondere auf der Basis von Edelstahl-oder Keramikkugeln. Jedoch läßt nur der Beschüß der Oberfläche mit Keramikperlen die notwendige Korrosionsbeständigkeit in simulierter physiologischer Lösung unverändert. Bei Einschränkung auf nichtmetallische Strahlmittel ist diese Nachbehandlung für eine Verminderung von Schäden durch Ermüdungsbrüche bzw. eine Erhöhung der Sicherheit von Implantaten bzw. Prothesen geeignet.
13 various hip joint and knee joint polyethylene components, previously removed from patients after an implantation period of 1.5 to 4 years, were tested and showed an obvious damage of the sliding surface. Macroscopic and microscopic investigations performed by light and scanning electron microscopy revealed an extreme wear of the polyethylene surface respectively an unregular wear at the component's border up to the mm-range. Additional specific X-ray and infrared spectrometric analyses proved in the clear majority of all cases the contribution of bone cement to increased abrasion and its influence upon the intensity of wear. Surplus bone cement which had not been completely removed from the prosthesis component or loosening of prosthesis components are considered as reason for inclusion of bone cement particles between the sliding surfaces.
Schlüsselwörter: Reibkorrosion, Spezialendoprothesen, Konusverbindung, Titanlegierung Das Problemkomplex der Reibkorrosion bei mehrteiligen Spezialprothesen wird aus der Sicht der Fügestellen betrachtet, wobei metallische Werkstoffe gleicher oder verschiedener Art in Kontakt stehen. Durch die körperliche Belastung können Mikrobewegungen stattfinden, die eine Zerstörung der Passivschicht bewirken und somit Korrosionsvorgänge einleiten können. So werden bei der Kombination von Teilen aus der Titanlegierung nach ISO 5832/3 Reibkorrosionsprozesse z. B. im Bereich einer typischen mechanischen Kegelverbindung beobachtet, die eine Beschichtung dieser Konstruktionselemente erforderlich macht. In dem Falle gibt dann die Auswertung des Reibkorrosionsstromes die Größenordnung der Bewegungen und damit verbundenen abgegebenen Stoffmenge an. Selbst für das Legierungselement Vanadin liegt der errechnete Anteil weit unterhalb der gesamten lebenswichtigen gespeicherten Menge im Körper. Interessanterweise findet bei der Kombination eines Teiles aus der Titanlegierung mit einem Teil aus der Kobaltlegierung nach ISO 5832/4, z. B. im Bereich einer Konusverbindung weder Kontakt-noch Spalt-oder Reibkorrosion statt.Das beschriebene Verhalten läßt sich z. T. mit der Abriebcharakteristik und dem Passivierungsverhalten von Titanwerkstoffen erklären, wonach eine gleichartige Paarung ohne geeignete Beschichtung ungünstig ist. Die angesprochene unterschiedliche Werkstoff-Paarung ist in dieser Hinsicht unbedenklich. This is a study of the problems of frictional corrosion in particular at the joint spots of special multiple component prostheses made of one or dif ferent metallic materials contacting each other. The physical load may effect micromotions causing a destruction of the passive layer which again may trigger corrosive action. As to the combination of components made of titanium alloy, according to ISO 5832/3 frictional corrosion is observed for example at a typic mechanical taper junction. Consequently coating of these components is necessary. In this case an Interpretation of the frictional corrosion flow reveals the magnitude of motions äs well äs the released material quantities. Even for the alloying element Vanadium the portion figured out is far below the totally accumulated and vital quantity. It is quite remarkable that there is neither contact nor crevice nor frictional corrosion for example at the cone junction when a titanium alloy component is combined with a cobalt alloy component according to ISO 5832/4. The described behaviour can be explained to some extent with the frictional characteristics and passivation behaviour of titanium materials according to which the combination of same materials without appropriate coating is disadvantageous. The herein mentioned combination of different materials is in this respect without danger.
The testing position for hip-joint prostheses proposed in DIN 58840 was compared with a testing position better adapted to the real stress situation, and possible effects studied. A load and movement analysis was carried out, and the respective position at which maximum impact loads occur was analysed. After the dynamic test, the results of the loads to be borne are presented for the testing position in accordance with DIN 58840 and that adapted to the real load in the case of two types of prostheses on the basis of Wöhler diagrams. The lateral stem stresses were then determined analytically. In the statistical assessment of the fracture analysis of prostheses broken in vivo, the directions of crack propagation were found to be in accordance with those of prostheses broken under dynamic loading, in particular in the case of the testing position adapted to the real stress situation.
