Summary: The serum concentration of several lipids, including high-density lipoprotein-cholesterol (HDL-C) and the HDL subfractions, HDL-2-C and HDL-3-C, were measured in 44 male and 26 female survivors of myocardial infarction and compared with those of a control group matched for age, sex, and body weight. Serum concentrations of total cholesterol (TC) and low-density lipoprotein (LDL-C) were significantly increased in patients as compared to control individuals. The total HDL-C concentration was lower in patients than in controls. By differential quantitation of HDL subfractions with a new precipitation method using polyethylene glycol, it was found that HDL-3-C was not significantly different between female patients and controls. The reduction of HDL-3-C in male patients was only of borderline significance. HDL-2-C in contrast was highly significantly reduced in both male and female patients. The greatest difference between patients and controls was found in the HDL-2/HDL-3-C ratio. It is therefore concluded that HDL-2-C quantitation is a valuable risk indicator for myocardial infarction yielding a better discrimination of patients from controls than total HDL-C quantitation.
The main cause for gastrointestinal mucosal lesions is H.P. infection (> 90%). A general mucoprotective therapy in patients with H.P. infection and NSAID therapy cannot be supported. It may be supposed that a part of mucosal lesions connected to NSAID-therapy in recent decades probably was the consequence of H.P. infections. Eradication of H.P. might be of higher importance in the future.
Die Bedeutung von Enzymen in der Therapie von rheumatischen ErkrankungenRheumatische Erkrankungen und Schmerz 5Mehr als 100 Millionen Menschen sind EU-weit von Erkrankungen des Stütz-und Bewegungsapparates betroffen, davon können rund 25% aufgrund ihrer rheumatischen Beschwerden bestimmte Alltagstätigkeiten nicht ausführen, die anderen Betroffenen nur in eingeschränktem Maß. Schmerz ist eines der Kardinalsymptome rheumatischer Erkrankungen, gleichgültig ob es sich um entzündliche, degenerative, metabolische oder infektiöse Prozesse handelt.Neben mechanischen Auslösern, wie z.B. erhöh-tem Gewebsdruck durch Ödeme spielen bei einer Vielfalt von Erkrankungen des rheumatischen Formenkreises primäre oder sekundäre Entzündun-gen in der Schmerzentstehung eine wesentliche Rolle. Enzyme steuern und beschleunigen biochemisch viele Lebensfunktionen und können aufgrund entzündungshemmender, ödemmindern-der, analgetischer und immunmodulierender Eigenschaften in der Therapie von Erkrankungen des rheumatischen Formenkreises eingesetzt werden.
In-vitro-Daten zu analgetischen Effekten von Enzymen 5Es gab schon relativ früh Beobachtungen, dass proteolytische Enzyme zu einer Spaltung und Inaktivierung von Entzündungsmediatoren wie Bradykinin und Prostaglandinen führen können [19,20,22]. Dies könnte bedeuten, dass durch Enzyme analgetische Effekte nicht nur durch eine Beeinflussung des Entzündungsgeschehens, sondern auch direkt am Nozizeptor erzielt werden können.Auch der messbare Abbau zirkulierender Immunkomplexe unter bestimmten Enzymgemischen [5,29] und die Reduktion von Adhäsionsmolekülen [31] sowie die Stimulation der Phagozytose [16] kann für eine Beeinflussung des schmerzhaften entzündlichen Schubes bei rheumatologischen Autoimmunerkrankungen von Bedeutung sein. Die Kombination von verschiedenen Enzymen, wie z.B. Bromelain, Trypsin und Rutin oder auch in Kombination mit Chymotrypsin und Papain hat sich bei Entzün-dungsprozessen als wirksamer im Vergleich zu Einzelenzymen herausgestellt [8, 21, 22].
Enzymtherapie bei Periarthritis humeroscapularis 5In der Rheumatologie ist die Periarthropathia humeroscapularis (PHS) eine mit Schulterschmerzen einhergehende Erkrankung. Übli-cherweise werden in der Behandlung der Periarthropathia humeroscapularis tendopathica nichtsteroidale Antirheumatika (NSAR), Analgetika und physikalische Therapie sowie lokal Glucocorticoid-Kristallsuspensionen eingesetzt.Das Potenzial möglicher Nebenwirkungen der genannten Pharmakotherapien ist bekannt. Diese führen nicht selten zu einem Therapieabbruch. Eine nebenwirkungsarme Therapie, wie die Enzymtherapie, könnte bei Auftreten von Nebenwirkungen konventioneller Therapieformen eine Alternative in der Behandlung der PHS darstellen, da die PHS in der Regel mit lokalen entzündlichen Vorgängen und Ödembildung einhergeht und diese im Wirkspektrum der PHS enthalten sind.In einer eigenen Pilotstudie wurde der analgetische Effekt einer Behandlung mit oralen Enzymen bei PHS geprüft.
