РЕФЕРАТВведение. Высокодозная химиотерапия (ВДХТ) с ауто-логичной трансплантацией гемопоэтических стволовых клеток (аутоТГСК) является эффективным методом ле-чения пациентов с лимфомой Ходжкина и агрессивными неходжкинскими лимфомами в тех случаях, когда стан-дартная химиотерапия в сочетании с лучевой не приводит к выздоровлению. ВДХТ и аутоТГСК также служат одним из основных методов лечения множественной миеломы. Между тем токсические эффекты трансплантации, в т. ч. и кардиотоксичность, могут значительно ухудшать про-гноз больных, получивших этот метод лечения. Цель. Оценить динамику изменений биохимических мар-керов кардиотоксичности (тропонина и N-терминального мозгового натрийуретического пропептида [NT-proBNP]) у пациентов со злокачественными лимфопролиферативными заболеваниями на фоне проведения ВДХТ и аутоТГСК. Материалы и методы. В исследование включено 157 па-циентов. Порог чувствительности методики при опреде-лении тропонина Т составлял 0,1 нг/мл, тропонина I -0,001 нг/мл (высокочувствительный тропонин). Тропонин Т был определен у 56 больных, тропонин I -у 101. Кон-центрацию тропонинов исследовали до начала кондицио-нирования, в Д0, Д+7 и Д+12. Уровень NT-proBNP опреде-ляли до начала кондиционирования, в Д0 и Д+12. Результаты. Повышение тропонина Т было зафиксировано у 2 (3,6 %) из 56 больных, тропонина I -у 27 (26,7 %) из 101 (p < 0,01). У всех пациентов при поступлении в стацио-нар уровень тропонинов был в пределах нормы. Тропонин Т повышался только в Д+7. Тропонин I повысился у 4 (4 %) больных в Д0, у 17 (16,8 %) -в Д+7 и у 11 (10,9 %) -в Д+12.
ABSTRACTBackground. High-dose chemotherapy (HDCT) with autologous hematopoietic stem cells transplantation (auto-HSCT) is an effective therapeutic option for patients with Hodgkin's lymphoma and aggressive non-Hodgkin's lymphomas in those cases, when the standard chemotherapy combined with the radiation therapy proves to be ineffective. The HDCT and auto-HSCT are also basic treatment options for multiple myeloma. However, toxic effects of the transplantation, including cardiotoxicity, may significantly worsen the prognosis of patients who receive this treatment. Aim. To evaluate changes in biochemical markers of cardiotoxicity (troponin and N-terminal prohormone of brain natriuretic peptide (NT-proBNP)) in patients with malignant lymphomas (receiving HDCT and auto-HSCT). Materials & Methods. 157 patients were enrolled in the study. The sensitivity threshold of the troponin T test was 0.1 ng/mL and troponin I 0.001 ng/mL (highly sensitive troponin). Troponin T (conventional troponin) was measured in 56 patients, troponin I was assessed in 101 patients. Serum troponin levels were evaluated before the conditioning, on D0, D+7, and D+12. The level of NT-proBNP was assessed before the conditioning, on D0 and D+12. Results.
