The German federal states initiated the "Pathological Gambling and Epidemiology" (PAGE) program to evaluate the public health relevance of pathological gambling. The aim of PAGE was to estimate the prevalence of pathological gambling and cover the heterogenic presentation in the population with respect to comorbid substance use and mental disorders, risk and protective factors, course aspects, treatment utilization, triggering and maintenance factors of remission, and biological markers. This paper describes the methodological details of the study and reports basic prevalence data. Two sampling frames (landline and mobile telephone numbers) were used to generate a random sample from the general population consisting of 15,023 individuals (ages 14 to 64) completing a telephone interview. Additionally, high-risk populations have been approached in gambling locations, via media announcements, outpatient addiction services, debt counselors, probation assistants, self-help groups and specialized inpatient treatment facilities. The assessment included two steps: (1) a diagnostic interview comprising the gambling section of the Composite International Diagnostic Interview (CIDI) for case finding; (2) an in-depth clinical interview with participants reporting gambling problems. The in-depth clinical interview was completed by 594 participants, who were recruited from the general or high-risk populations. The program provides a rich epidemiological database which is available as a scientific use file.
Fragestellungen: Welche psychische Komorbidität liegt bei stationär behandelten pathologischen Glücksspielern vor, welches zeitliche Muster des Auftretens und welche Erkrankungswahrscheinlichkeit zeigt sich? Lassen sich Subtypen von Glücksspielern unterscheiden? </p><p> Methode: 101 Glücksspieler in stationärer Behandlung wurden mit dem DIA-X, der IPDE und der SCL-90-R untersucht. </p><p> Ergebnisse: Die höchsten Komorbiditätsraten (Zwölfmonatsprävalenz) zeigen sich für affektive Störungen (51,5 %), Angststörungen (47,5 %), Persönlichkeitsstörungen (27,7 %) und substanzbezogene Störungen (25,7 %). Angststörungen treten überwiegend (76,9 %) vor dem pathologischen Glücksspielen auf, affektive Störungen danach (60,6 %). Die Erkrankungswahrscheinlichkeit für Pathologische Glücksspieler ist erhöht. Es lassen sich drei Subtypen differenzieren. </p><p> Schlussfolgerungen: Je nach Art der Komorbidität ist eine differenzierte Behandlungsstrategie notwendig.

The infl uence of comorbid psychic disorders, characteristics of the former career of gambling and sociodemographic factors on treatment results as well as on the further development of the pathological gambling disorder was evaluated. Methods: Characteristics of the previous disorder course, comorbid axis I und axis II disorders and sociodemographic marks were measured. To evaluate the treatment results and the further development of the pathological gambling disorder the patients completed an assessment for psychological distress and for involvement in gambling. In addition abstinence at six-monthfollow-up was investigated. Data basis: 101 inpatients of a clinic for psychosomatic and dependence disorders with a primary diagnosis of pathological gambling, who were treated in a gambling abstinence focused psychotherapy program, participated in the study. A pre-post-and six-month-follow-up assessment was conducted. Results: The prevailing form of gambling was the one at slot machines (94.1 % ) followed by casino games (17.8 % ) and sports-betting (12.9 % ). The active gambling time was at average 14.5 years and after an average of 5.8 years the patients felt addicted to gambling. The sample in general demonstrated a signifi cant decrease of psychological distress and of involvement in gambling from pre to post and from pre to follow-up assessment. Disadvantageous infl uences on treatment results are comorbid anxiety disorders, personality disorders, lack of gambling abstinence before joining the treatment program and the lack of a partnership.
Fragestellung: Es wurde anhand eines Entlassungsjahrganges stationär behandelter pathologischer Glücksspieler untersucht, wie hoch die katamnestische Erfolgsquote, definiert als Glücksspielfreiheit zum Nachbefragungszeitpunkt, ist. Weiter wurden die Anlässe für erneutes Glücksspielen und Veränderungen der Involvierheit in das Glücksspielen untersucht. Ferner wurde der Frage nachgegangen, wie sich die Lebenssituation der Betreffenden entwickelt hat, und ob sich spezifische Risikofaktoren für die Aufrechterhaltung von Glücksspielfreiheit bestimmen lassen. Methodik: Erhoben wurden soziodemographische Merkmale, Kennzeichen des Störungsverlaufes, die gedankliche und emotionale Involviertheit in das Glücksspielen, komorbide Störungen sowie Behandlungsmerkmale. Datenbasis: Untersucht wurden 617 Patienten, die zwischen dem 01. 10. 2009 und dem 30. 09. 2010 mit der Zuweisungsdiagnose Pathologisches Glücksspielen in fünf Fachkliniken für Psychosomatik und Abhängigkeitserkrankungen behandelt wurden. Die Messungen fanden statt bei Aufnahme (T1), bei Entlassung (T2) und 12 Monate nach der Behandlung (T3). Ergebnisse: Die Rücklaufquote lag bei 55,9 % (N = 345). Die katamnestische Erfolgsquote lag berechnet auf Basis der Gesamtstichprobe bei 39,7 % und berechnet auf Basis der Katamneseantworter bei 71,0 %. Die soziale und ökonomische Situation hat sich für einen erheblichen Teil der Befragten verbessert. Als Risikofaktoren für die Aufrechterhaltung von Glücksspielfreiheit erwiesen sich: Eine geringe Fähigkeit zum Umgang mit negativ getönten Affekten, eine hohe Anzahl von Spieltagen pro Monat, eine hohe kognitive und emotionale Involviertheit in das Glücksspielen, eine geringe Anzahl glücksspielfreier Tage vor Behandlungsbeginn, sowie das Vorliegen von Arbeitslosigkeit. Schlussfolgerungen: Die Rücklaufquote kann als gut bewertet werden. Die katamnestischen Erfolgsquoten sind vergleichbar denen aus der Behandlung alkoholabhängiger Patienten. Sie liegen über denjenigen, die von ambulanten Behandlungsprogrammen berichtet werden. In zukünftigen Katamnesestudien sollten die bisher identifizierbaren Risikofaktoren für die Aufrechterhaltung von Glücksspielfreiheit genauer untersucht werden. Weiter sollten Wege gefunden werden, die Rücklaufquoten noch zu verbessern.
ZusammenfassungIn den letzten Jahren haben sich die Möglichkeiten, Sportwetten zu betreiben, in Deutschland erheblich ausgeweitet und diversifiziert. Korrespondierend dazu ist zu erwarten, dass die Anzahl der Menschen, die Probleme durch Sportwetten aufweisen, steigen wird. Vor diesem Hintergrund wird im vorliegenden Beitrag beschrieben, welche besonderen Herausforderungen die Behandlung pathologisch Glücksspielender mit sich bringt, die bevorzugt an Sportwetten teilnehmen. Dies geschieht vor dem Hintergrund der klinischen Behandlungserfahrung und der Berichte von Patienten und Patientinnen aus der MEDIAN Klinik Schweriner See. In dem Beobachtungszeitraum von 2008 bis 2017 stieg unter den Betreffenden, die sich wegen Glücksspielen in Behandlung begaben, der Anteil derjenigen, die ausschließlich oder zu einem erheblichen Anteil Online-Sportwetten praktizierten, von 3,4 auf 13,2%. Die Charakteristika von Patienten und Patientinnen mit exzessivem Sportwetten werden herausgearbeitet. Darauf aufbauend wird aufgezeigt, welche besonderen Herausforderungen und Spezifika sich daraus für die Behandlung ergeben. Dies geschieht exemplarisch anhand der Therapiebausteine des in der MEDIAN Klinik Schweriner See praktizierten Behandlungsprogramms. Es zeigt sich, dass die subjektive Überzeugung, das Wettgeschehen beherrschen und kontrollieren zu können, besonders hartnäckig ist. Durch die Verbindung von Sportbegeisterung und Wettanreiz wird zudem ein besonders intensives und langanhaltendes positives Spannungsgefühl erreicht. In der Therapie muss insbesondere auf die Verwobenheit der Auslöser für das Sportwettverlangen mit alltäglichen Lebensvollzügen geachtet werden. Wegen der sensorischen Nähe von digitalem Wettverhalten via PC oder Smartphone zum Online-Banking sind hier im Rahmen des therapeutischen Geldmanagements spezifische Strategien zu entwickeln.
