Obsessive-compulsive features are common in schizophrenia. Current genetic, neurobiological, clinical and epidemiological data about obsessive-compulsive symptoms/disorder in schizophrenia suppose that schizophrenia combined with OCD is more than a mere comorbidity. Epidemiologic and neurobiological evidence suggest that the patients with comorbid OCD and schizophrenia may present a special category among patients with schizophrenia. Although a specific profile has not been emerged yet; clinical, neurobiological research about schizo-obsessive patients have been extensively pursued.
The aim of this study was to assess impulsivity and attention deficit hyperactivity disorder in adult patients with trichotillomania. Materials and Methods: Subjects diagnosed with trichotillomania primary or comorbid to other psychiatric disorders who met the inclusion criteria were enrolled in the study. Data were collected retrospectively. Minnesota Impulse Control Disorders Interview Scale (MIDI), the Barratt's Impulsivity Scale (BIS), Adult ADD/ADHD DSM IV-Based Diagnostic Screening and Rating Scale, Wender Utah Rating Scale (WURS) were evaluated. Results: 40 patients with trichotillomania and 18 patients with trichotillomania + adult attention deficit hyperactivity disorder were included in the study. Sociodemographic characteristics of two groups were similar. Rate of pathologic gambling was higher in trichotillomania + adult attention deficit hyperactivity disorder group. Trichotillomania + adult attention deficit hyperactivity disorder group had statistically higher scores from attention deficit part of Adult ADD/ADHD DSM IV-Based Diagnostic Screening and Rating Scale. Conclusion: Adult attention deficit hyperactivity is more related to attention deficit in trichotillomania patients and impulsivity is a common feature in both disorders. Amaç: Bu çalışmada trikotillomani hastalarında dürtüsellik ve erişkin dikkat eksikliği ve hiperaktivite bozukluğunu değerlendirmeyi amaçladık. Gereç ve Yöntem: Birincil ya da eş tanı olarak trikotillomani tanısı konulan ve dahil edilme kriterlerine uygun olan hastalar çalışmaya dahil edildi. Veriler retrospektif olarak toplandı. Minnesota Dürtü Kontrol Bozuklukları Görüşmesi (MIDI), Barratt İmpulsivite Ölçeği (BİÖ), Erişkin Dikkat Eksikliği Hiperaktivite Bozukluğu tanı ve değerlendirme envanteri, Wender Utah Derecelendirme Ölçeği (WUDÖ) uygulandı. Bulgular: Çalışmaya trikotillomani tanısı olan 40 ve erişkin dikkat eksikliği hiperaktivite bozukluğu + trikotillomanisi olan 18 hasta dahil edildi. Her iki grubun sosyodemografik özellikleri birbirine benzerdi. Patolojik kumar oynama erişkin dikkat eksikliği hiperaktivite bozukluğu + trikotillomani grubunda daha sıktı. Erişkin dikkat eksikliği hiperaktivite bozukluğu + trikotillomani grubu, erişkin dikkat eksikliği hiperaktivite bozukluğu tanı ve değerlendirme envanterinin dikkat eksikliği alt ölçeklerinden istatistiksel olarak anlamlı düzeyde daha yüksek puan aldı.. Sonuç: Trikotillomani hastalarında erişkin dikkat eksikliği ve hiperaktivitenin daha çok dikkat eksikliğiile ilişkili olduğunu, dürtüselliğin ise her iki bozuklukta ortak bir özellik olduğunu göstermiştir.
ÖzPurpose: The aim of this study was to investigate the frequency of attention deficit and hyperactivity disorder (ADHD) and impulsivity in patients with compulsive buying disorder (CBD). Materials and Methods: The medical records of 82 patients admitted to the Psychiatry Department of Cukurova University Medical School Balcalı Hospital and diagnosed as CBD were investigated retrospectively. Patients' Turkish version of the Structured Clinical Interview for DSM-IV (SCID-I), modified Turkish version of the Minnesota Impulse Disorders Interview (MIDI), Turkish version of the Barratt Impulsiveness Scale Version 11 (BIS-11) and adult ADD/ADHD diagnosis and evaluation inventory data were evaluated. Results: 50% (n=41) of the patients had at least comorbidity of one diagnosis of impulse control disorder not elsewhere classified. 42.7% (n=35) of the patients had a diagnosis of ADHD in childhood and 30.5% (n=25) in adulthood. In comparison of BIS-11 subscale scores of patients with ADHD and without ADHD, patients with the diagnosis of ADHD had statistically significant higher scores for BIS-11 subscales except motor impulsivity subscale. Conclusion: CBD is commonly seen in women, often accompanied by ADHD and other impulse control disorders. When the diagnoses of CBD and ADHD are overlapped, it is seen that features such as general impulsivity, attention deficit and lack of planning are more evident. Amaç: Bu çalışmada kompulsif satın alma bozukluğu (KSAB) tanısına sahip bireylerde dikkat eksikliği hiperaktivite bozukluğunda (DEHB) sıklığı ve KSAB ile dürtüsellik ilişkisinin araştırılması amaçlanmıştır. Gereç ve Yöntem: Çukurova Üniversitesi Tıp Fakültesi Balcalı Hastanesi Ruh Sağlığı ve Hastalıkları Anabilim Dalı'na başvuran ve KSAB tanısı konan 82 hastanın tıbbi kayıtları retrospektif olarak incelendi. Hastalara ait DSM-IV (Diagnostic and Statistical Manual of Mental Disorders IV) Eksen I Bozuklukları İçin Yapılandırılmış Klinik Görüşme Ölçeği (SCID-I), Minnesota Dürtü Kontrol Bozuklukları Görüşme Ölçeği (MIDI), Barratt Dürtüsellik Ölçeği-11 (BİS-11) ve Erişkin DEB/DEHB Tanı ve Değerlendirme Envanteri verileri değerlendirildi. Bulgular: Hastaların %50'sinde (n=41) KSAB'a komorbid en az bir başka türlü adlandırılamayan (BTA) dürtü kontrol bozukluğu (DKB) tanısı mevcuttu. Hasta grubunun %42.7'sinde (n=35) çocukluk döneminde, %30.5'inde (n=25) erişkin dönemde DEHB tanısı mevcuttu. DEHB tanısı olan hastalarla, DEHB tanısı olmayan hastaların BİS-11 ölçek puanlarının karşılaştırmasında; motor dürtüsellik alt ölçek puanı dışında DEHB tanısı olanların istatistiksel olarak daha yüksek puanlar aldıkları görüldü. Sonuç: KSAB daha çok kadınlarda görülen, DEHB ve diğer dürtü kontrol bozukluklarının sıklıkla eşlik ettiği bir ruhsal bozukluktur. KSAB ve DEHB tanıları birlikte görüldüğünde genel dürtüsellik, dikkatsizlik ve plan yapmama gibi özelliklerin daha da belirginleştiği görülmektedir.
