The interview has a semistructured format. which allowsa flexible use of standardquestions, probesand statements about family life. The SCFI focuses on the family. while encouraging family members to interact spontaneously. The interview consistsof four phasesincluding topicssuch as family togetherness. areas of conflictand disagreement. discipline and decisionmaking.and issuesof roles and responsibilities. The family interviews were video-recorded and later used for the rating of EE. This was conducted by following the same rules as for the ratingof EE usingthe CFI (Leftand Vaughan. 1985; Vaughan and Left. 1976). The subjects in this study consisted of 53 families in two main groups: 41 families with an anorexic patient and 12 families with a bulimic patient. Our eventual sample of 79 relatives consistedof 40 mothers.27 fathers. and 12 husbands. All patients were women and their meanage was 26 years (range= IS-45 years). The results indicate that the levels of Critical Comments (CC), Hostility (HOS), Emotional Over-Involvement (EOI) and Positive Remarks(PR) were rated low (e.g. 16 families (30%) made no CC and 17 families (26%) made only one CC). The relatives were rated as being moderately warm in the way they related to the patientduring the interview. A comparison betweenmothersandfathers showed mothersto be significantly moreoverinvolved thanfathers(t=-3.68. df = 25, P < 0.001). Mothers also scored significantly higher on PR (t =-2.78. df =25. P < 0.01) and Warmth (t =-2.56, df =25, P < 0.01) than did fathers. There was no significant difference among relatives in theirlevelof CC and also betweenanorexics and bulimics in their level of EE index.The clinical and research implications for these findings are discussed.
RESUMEN-El objetivo de este estudio es el de investigar la dependencia del alcohol en soldados profesionales de combate con Trastorno de Estrés Postraumático (PTSD) relacionado con el incremento de la agresión. Se entrevistaron dos grupos de soldados de combate profesionales, utilizando entrevistas clínicas estructuradas basadas en el criterio DSM IV para PTSD crónico (N = 40), y PTSD coexistente con dependencia al alcohol (N = 40). La escala de ratios para la agresión A-87 se utilizó para explorar el nivel de agresión entre estos grupos. Los soldados profesionales de combate con PTSD comórbido con una dependencia al alcohol, mostraron mayores niveles de agresión verbal que aquellos soldados que presentaban únicamente PTSD, (54,97+-8,62 vs 49,47 +-12,76; t = 2,19; p < 0,05), así como conductas manifiestas de agresión física (41,75+-9.80 vs 35,62 +-10,80; t = 2,96; p < 0,05). La comorbilidad con dependencia al alcohol podría jugar un papel en cuanto a los niveles de agresión en las experiencias de PTSD crónicos especialmente al presentarse la agresión.
RESUMEN -El objetivo de este estudio fue investigar: la prevalencia de dependencia alcohólica actual, la anamnesis familiar de la adicción alcohólica, las características sociales y demográficas, el número de hospitalizaciones y el grado de cumplimiento con la psicofarmacoterapia, como predictores de mala evolución de la esquizofrenia comórbida con la adicción alcohólica. Nuestros resultados muestran que en los varones con esquizofrenia las tasas de prevalencia de adicción alcohólica comórbida son más altas que en mujeres.La comorbilidad de la adicción alcohólica y la esquizofrenia en hombres se relaciona con mayores tasas de divorcio y con la edad. En mujeres, la comorbilidad adicción alcohólica y esquizofrenia se relaciona con un nivel bajo de educación, una tasa de solteras más alta y con seguro de enfermedad familiar. Hay una fuerte evidencia de adicción alcohólica en familiares de primer grado de esquizofrénicos con adicción alcohólica comórbi-da, tanto hombres como mujeres. La adicción alcohólica comórbida con esquizofrenia en hombres y mujeres se relaciona con una mala cumplimentación del tratamiento psicofarmacológico y con tasas más altas de hospitalización, comparada con esquizofrenia sola.
RESUMEN -Los acontecimientos traumáticos que sobrepasan la experiencia humana normal, tales como las situaciones que ponen en peligro la vida o la integridad física o la exposición de personas próximas a un peligro similar, pueden producir determinados problemas psicológicos en el individuo, que se describen en el marco de un trastorno denominado Trastorno de Estrés Postraumático. La exposición a numerosos acontecimientos e influencias estresantes produce, como resultado de numerosos factores, el que el organismo desencadene una serie de mecanismos de defensa, en los que juega un papel fundamental el Sistema Nervioso Central, pero también el sistema suprarrenal y el cardiorespiratorio. Cuando la homeostasis interna está en peligro, se activa el sistema endocrino y su respuesta al peligro depende de la intensidad del agente causante del estrés, de la duración de la exposición y de la capacidad del sistema para compensar el estrés. La exposición al estrés y el efecto directo del agente causante de estrés desencadena además otro mecanismo de defensa, cual es el aumento del consumo de alcohol a modo de automedicación y bajo la influencia directa del agente causante de estrés. Tanto el PTSD como el alcoholismo conducen de forma gradual a la afectación del sistema neuroendocrino. En nuestro estudio hemos incluido dos grupos de pacientes, concretamente el grupo de pacientes que padecen de PTSD y de alcoholismo y el grupo de pacientes que padecen únicamente de PTSD. Hemos llevado a cabo la evaluación de los niveles de cortisol en suero y en la orina de 24 horas, el test de supresión de la dexametasona y los niveles en suero de ACTH, TSH, T3, T4, testosterona, prolactina y hormona del crecimiento.Los resultados obtenidos llevan a considerar la presencia de alteraciones en los ejes neuroendocrinos en los dos grupos de esta investigación, alteraciones que se presentan más marcadas en el grupo de sujetos que presentan comorbilidad de PTSD y alcoholismo.
