Descrever a incidência e impacto nos desfechos de infecções pulmonares necrosantes (IPN) em pacientes com diagnóstico de pneumonia associada a ventilação mecânica (PAV) em indivíduos com suspeita e/ou confirmação de COVID-19 em uma Unidade de Terapia Intensiva (UTI). Estudo observacional retrospectivo na UTI em um hospital referência COVID-19 no litoral norte de São Paulo, no período de abril de 2020 a agosto de 2021. Foram incluídos no estudo pacientes com diagnóstico de PAV segundo os critérios estabelecidos pela Agência Nacional de Vigilância Sanitária (ANVISA) e com presença de cavitações em tomografia computadorizada de tórax, caracterizando pneumonia necrotizante, abscesso e/ou gangrena pulmonar. Nossa população de estudo foi constituída por 104 pacientes com diagnóstico de PAV, sendo 6 (5,7%) destes pacientes caracterizados como IPN. A taxa de mortalidade de todos os casos de PAV foi 59,52%, sendo que dentre os caso de IPN, a taxa de mortalidade foi de 50%. Dentre os agentes identificados em todos casos de PAV, houve predomínio de bactérias gram-negativas (69,3%). Os principais agentes foram o Acinetobacter baumanii (37,8%), Staphylococco aureus (23,5%), Pseudomonas auriginosa (22,4%) e Klebsiela pneumoniae (7,1%). Das IPN, 83,3% foram causados por Acinetobacter baumanii. O tempo médio de internação dentre os paciente com PAV sem presença de IPN foi de 30,54 dias e dos pacientes com IPN foi de 65,66 dias. Como demonstrado em nossos resultados a IPN são potenciais complicações em pacientes com COVID-19 corroborando as características de comprometimento vascular e infeccioso comum da infecção viral e bacteriana secundária, podendo impactar principalmente no tempo de permanência hospitalar.
Introdução: As cardiopatias congênitas são uma das principais causas de morte na infância. À medida que houve evolução no processo de diagnóstico dessas malformações e também nos processos cirúrgicos para o reparo das mesmas, as complicações desencadeadas no pós-operatório também se tornaram mais evidentes. Devido ao heterogêneo quadro dessas complicações, principalmente respiratórias, a fisioterapia possui uma atuação de muita importância, buscando gerar conforto e bem estar aos pacientes. Objetivo: Avaliar a atuação da fisioterapia nas possíveis complicações respiratórias no período pré e pós-operatório em crianças cardiopatas congênitas que realizaram cirurgias de correção. Métodos: Revisão de literatura, cuja pesquisa é direcionada para tratamentos fisioterapêuticos nas complicações respiratórias em crianças com cardiopatia congênita. Resultados: Foram pesquisados 101 artigos, mas apenas 25 foram utilizados nesse trabalho. Conclusão: Existem diferentes manejos realizados pela fisioterapia e diferentes opiniões quanto aos resultados de determinadas técnicas. Mesmo diante de tais divergências, a fisioterapia tem importante atuação tanto na prevenção quanto no tratamento de possíveis complicações respiratórias no pós-operatório.Palavras-chave: exercícios respiratórios, cardiopatia, Fisioterapia.
