Electrical stimulation is commonly used for strengthening muscle but little evidence exists as to the optimal electrode size, waveform, or frequency to apply. Three male and three female subjects (22-40 years old) were examined during electrical stimulation of the quadriceps muscle. Two self adhesive electrode sizes were examined, 2 cm x 2 cm and 2 cm x 4 cm. Electrical stimulation was applied with square and sine waveforms, currents of 5, 10 and 15 mA, and pulse widths of 100-500 micros above the quadriceps muscle. Frequencies of stimulation were 20, 30, and 50 Hz. Current on the skin above the quadriceps muscle was measured with surface electrodes at five positions and at three positions with needle electrodes in the same muscle. Altering pulse width in the range of 100-500 micros, the frequency over a range of 20-50 Hz, or current from 5 to 15 mA had no effect on current dispersion either in the skin or within muscle. In contrast, the distance separating the electrodes caused large changes in current dispersion on the skin or into muscle. The most significant finding in the present investigation was that, while on the surface of the skin current dispersion was not different between sine and square wave stimulation, significantly more current was transferred deep in the muscle with sine versus square wave stimulation. The use of sine wave stimulation with electrode separation distances of less then 15 cm is recommended for electrical stimulation with a sine wave to achieve deep muscle stimulation.
RESUMEN
El presente artículo describe y analiza el rol que ha jugado la gobernanza en el desarrollo del modelo comunitario de salud mental en Chile. Se describen y analizan cinco dimensiones de la gobernanza: 1) el desarrollo de una serie de estrategias para favorecer la participación de los usuarios de servicios de salud mental ha logrado resultados insuficientes; 2) la implementación de tres planes nacionales de salud mental, que han mantenido la misma visión estratégica a lo largo de tres décadas, han contribuido a aumentar la accesibilidad a la atención comunitaria de las personas con enfermedades mentales y disminuir el rol de los hospitales psiquiátricos; 3) la inexistencia de una ley de salud mental persiste aún como un desafío, aunque haya sido parcialmente compensada por distintas leyes y decretos; 4) las regulaciones en la asignación de los recursos financieros, la formación del recurso humano y las directrices técnicas han contribuido a mejorar el acceso y la calidad de la atención; y 5) la construcción progresiva de un sistema de información en salud mental ha permitido conocer principalmente las tasas de acceso, las brechas de tratamiento y las inequidades geográficas. La experiencia chilena confirma la importancia de aplicar un conjunto de estrategias y mecanismos de gobernanza, para favorecer el desarrollo y el funcionamiento de los servicios de salud mental. El tipo de estrategias y mecanismos de gobernanza podrán variar en los distintos países según los contextos y las ventanas de oportunidades.
In the present study, we examined ethnic variables (viz., multigroup ethnic identity and other group orientation) along with negative life events as predictors of depressive symptoms and suicidal behaviors in a sample of 156 (38 male and 118 female) Latino college students. Results of conducting hierarchical regression analyses indicated that the set of ethnic variables accounted for significant variance in both depressive symptoms and suicidal behaviors. The inclusion of negative life events was found to account for significant additional variance in both outcomes, beyond what was accounted for by ethnic variables. Interestingly, beyond main effects, results indicated a significant Other Group Orientation × Negative Life Events interaction effect to account for additional variance in depressive symptoms and suicidal behaviors. Some implications of the present findings are discussed, including the potential value for Latinos to broaden their attitudes and interactions with other ethnic groups.
