RESUMENIntroducción Se desarrolló una investigación operativa en octubre de 2006, que en breve tiempo aportó información útil para la toma de decisiones en el trabajo de erradicación del mosquito Aedes aegypti. Objetivos Caracterizar la situación organizacional existente en los Grupos de Control de Vectores de la provincia Ciudad de La Habana, la llamada "Campaña" e identificar aquellos factores que obstaculizaban el trabajo. Métodos Se realizó un estudio descriptivo y transversal. Se trabajó con una muestra intencional que incluyó 278 trabajadores de diferentes categorías ocupacionales y procedentes de 15 áreas de salud, seleccionadas a partir de una estratificación de la provincia por criterio cualitativo. Se estudiaron también 57 directivos. Se exploraron las variables tiempo en la actividad y otras relacionadas con el propósito de la organización, estructura organizativa, capacitación, relaciones entre trabajadores y con los superiores en rango (jefes), dentro y fuera del sector; Revista Cubana de Salud Pública. 2009; 35(2) http://scielo.sld.cu 2 mecanismos de apoyo, uso y formas de reconocimiento, condiciones de trabajo y recursos. Además, se evaluó el clima organizacional a partir de 16 criterios en las dimensiones: Liderazgo, Motivación, Reciprocidad y Participación. Se emplearon técnicas cuantitativas y cualitativas: encuesta, entrevista a profundidad y grupo focal, y se trianguló la información obtenida. Resultados Los principales resultados estuvieron en la inadecuada selección del personal y poco tiempo en la actividad, insuficiente integración de los operarios al trabajo del Equipo Básico de Salud no inherente a la estructura organizativa, inadecuadas normas de trabajo que propiciaban conductas fraudulentas, insuficiencias en los mecanismos de apoyo, de estimulación y reconocimiento que generaban desmotivación en los trabajadores; estilos de dirección que dificultaban la comunicación y las relaciones interpersonales y métodos de dirección poco participativos.Conclusiones Se localizan numerosos y diversos problemas de tipo organizacional que influyen desfavorablemente en el clima laboral y en consecuencia limitan la dinámica de trabajo en los grupos estudiados. RESUMENCon el desarrollo de la humanidad, y en particular con los avances en el terreno de la medicina, es necesario hacer un llamado para humanizar la asistencia en salud y ofrecer servicios de mayor calidad. La calidad en la atención médica debe estar basada en actividades encaminadas a garantizar los servicios de salud accesibles y equitativos con profesionales óptimos y teniendo en cuenta los recursos disponibles, logrando la satisfacción del usuario con la atención recibida. El presente trabajo tiene como objetivos reflexionar sobre la necesidad de integración de elementos de carácter técnico y también de procesos, objetivos y subjetivos, involucrados en el fenómeno de la calidad y enfatizar en su elemento subjetivo: la satisfacción, que representa la vivencia subjetiva derivada del cumplimiento o incumplimiento de las expectativas que tiene un sujeto con respecto a algo. The present paper was aimed at reflecting on the need of integration of technical elements and objective and subjective processes involved in quality and at putting emphasis on its subjective element, that is, the satisfaction that represents the subjective experience derived from the met or unmet expectations of an individual. INTRODUCCIÓNEn las últimas décadas del siglo xx las transformaciones sociales y económicas han producido cambios en la salud de la población y, por tanto, en las políticas de prestación de salud a escala mundial. En tales condiciones se promueve una atención de alta calidad centrada en el usuario o cliente.En la asistencia en salud, el llamado es hacia su humanización y a ofrecer servicios de mayor calidad. El concepto de calidad en salud debemos enmarcarlo según Vanormalingen (1996) en cinco elementos fundamentales: excelencia profesional, uso eficiente de los recursos, mínimo riesgo para el paciente, alto grado de satisfacción, impacto final que tiene en la salud.
