Context Over the past century, nursing homes and hospitals increasingly have become the site of death, yet no national studies have examined the adequacy or quality of end-of-life care in institutional settings compared with deaths at home.
The purpose of this study was to examine the reliability and validity of the Toolkit After-Death Bereaved Family Member Interview to measure quality of care at the end of life from the unique perspective of family members. The survey included proposed problem scores (a count of the opportunity to improve the quality of care) and scales. Data were collected through a retrospective telephone survey with a family member who was interviewed between 3 and 6 months after the death of the patient. The setting was an outpatient hospice service, a consortium of nursing homes, and a hospital in New England. One hundred fifty-six family members from across these settings participated. The 8 proposed domains of care, as represented by problem scores or scales, were based on a conceptual model of patient-focused, family-centered medical care. The survey design emphasized face validity in order to provide actionable information to health care providers. A correlational and factor analysis was undertaken of the 8 proposed problem scores or scales. Cronbach's alpha scores varied from 0.58 to 0.87, with two problem scores (each of which had only 3 survey items) having a low alpha of 0.58. The mean item-to-total correlations for the other problem scores varied from 0.36 to 0.69, and the mean item-to-item correlations were between 0.32 and 0.70. The proposed problem scores or scales, with the exception of closure and advance care planning, demonstrated a moderate correlation (i.e., from 0.44 to 0.52) with the overall rating of satisfaction (as measured by a five-point, "excellent" to "poor" scale). Family members of persons who died with hospice service reported fewer problems in each of the six domains of medical care, gave a higher rating of the quality of care, and reported higher self-efficacy in caring for their loved ones. These results indicate that 7 of the 8 proposed problem scores or scales demonstrated psychometric properties that warrant further testing. The domain of closure demonstrated a poor correlation with overall satisfaction and requires further work. This survey could provide information to help guide quality improvement efforts to enhance the care of the dying.
Recalled body weight and self-reported current weight were validated in a longitudinal study population by comparing recalls at 50 y to actual measures taken at ages 18, 30, 40, and 50 y. Recalled body weights were also compared with reported desired weights at these same ages. Self-reported weights at 50 y were equally accurate for both males and females; the mean reporting underestimate was -1.98 kg for males and -1.86 kg for females. Males' self reports at age 50 y were influenced by years of education (P less than 0.005) and current body size (P less than 0.0001) whereas females' were not. Correlations between recall of past weights and measured weights ranged from r = 0.87 at 18 y to 0.95 at 40 y. Recalls of past body weight were not significantly influenced by the passage of time, the number of years of education, or the accuracy of current weight reports. Current body size (wt/ht2) was significantly associated with life-time weight dissatisfaction in both sexes (P less than 0.0005).
The validity of long-term recall and current assessments of height, weight, and fatness relative to peers was investigated among 91 middle-aged participants in a longitudinal growth study. The recollections of 50-year-old participants concerning perceived body size in comparison to peers during childhood (aged 5-7 years), adolescence (aged 10-18 years), and at ages 30, and 40 years were compared with physical measurements taken at these times. Correlations between perceived and actual body size at all ages from childhood through middle-age were moderate but significant (P less than 0.005) and were influenced by gender and phases of physical growth (early and late adolescence). In general, accuracy of self-reports of current body size were not significantly better than recalls of body size up to 50 years earlier. Respondents' recall of various physiological events was also assessed. Females' actual and recalled year of menarche were correlated (r = 0.67; P less than 0.0001). Age at menarche was recalled within 1 year of the actual event by 84% of the females. Fifty percent of both sexes recalled their year of maximal growth in height within 1 year and recalled the timing of their maturation (early, average, or late) in relationship to their peers equally well (P less than 0.001).
Purpose To examine associations of milk, yogurt, cheese, cream, most dairy (total dairy without cream) and fluid dairy (milk+yogurt) with bone density (BMD) at femoral neck (FN), trochanter (TR) and spine, and with incident hip fracture over 12-y follow-up in the Framingham Offspring Study. Methods 3,212 participants completed a food frequency questionnaire (1991–1995 or 1995–1998) and were followed for hip fracture until 2007. 2,506 participants had DXA BMD (1996–2001). Linear regression was used to estimate adjusted mean BMD while Cox-proportional hazards regression was used to estimate adjusted hazard ratios (HR) for hip fracture risk. Final models simultaneously included dairy foods adjusting for each other. Results Mean baseline age was 55 (±1.6)y, range: 26–85). Most dairy intake was positively associated with hip and spine BMD. Intake of fluid dairy and milk were related with hip but not spine BMD. Yogurt intake was associated with TR-BMD alone. Cheese and cream intakes were not associated with BMD. In final models, yogurt intake remained positively associated with TR-BMD, while cream tended to be negatively associated with FN-BMD. Yogurt intake showed a weak protective trend for hip fracture [HR(95%CI): ≤4 serv/wk: 0.46 (0.21–1.03) vs. >4 serv/wk: 0.43 (0.06–3.27)]. No other dairy groups showed a significant association (HRs range: 0.53–1.47) with limited power (n, fractures=43). Conclusion Milk and yogurt intakes were associated with hip but not spine BMD, while cream may adversely influence BMD. Thus, not all dairy products are equally beneficial for the skeleton. Suggestive fracture results for milk and yogurt intakes need further confirmation.
