Introdução: O câncer do colo do útero é uma doença prevalente e 60% dos casos registrados no Brasil são diagnosticados em estágios avançados. A radioterapia associada a quimioterapia é o tratamento de eleição, e existe uma elevada taxa de recidiva. As possibilidades terapêuticas para recidivas são limitadas com apenas 20% de sobrevida em 12 meses. Neste contexto a reirradiação com braquiterapia intersticial com alta taxa de dose (HDR-IB) pode ser uma alternativa, embora existam poucas informações. Objetivo: Avaliar a resposta clínica, a sobrevida pós-recidiva, sobrevida livre de doença (SLD) e a toxicidade com a utilização de reirradiação com HDR-IB em mulheres com recidiva pélvica de carcinoma do colo do útero previamente irradiado. Metodologia: estudo de coorte retrospectivo com 45 mulheres com recidiva pélvica de carcinoma do colo do útero previamente irradiados e submetidas à HDR-IB no período entre novembro de 1998 e dezembro de 2014, com seguimento até 2018, no Hospital da Mulher, CAISM, Unicamp. Foram consideradas informações clínicas, da neoplasia, do tratamento primário, relacionadas com a técnica da HDR-IB, utilização ou não de ultrassonografia para inserção de agulhas e a toxicidade. A análise estatística foi realizada através dos testes Qui Quadrado ou Fisher, montadas curvas de sobrevida pelo método de Kaplan-Meier e analisadas pelo teste log-rank, seguido de análise de regressão de Cox. Foi considerado um p<0,05 para significância estatística. Resultados: 71% das mulheres tinham menos de 65 anos na recidiva e 62% dos casos eram carcinoma espinocelular. O tempo médio decorrido entre o tratamento inicial até a recidiva foi de 35,6 meses. Foi observado 67% (30/45) de resposta clínica completa com a HDR-IB, 12 permaneceram vivas sem doença, oito vivas com doença e 10 óbitos registrados. Entre as 15 mulheres sem resposta clínica completa, a sobrevida foi curta, até oito meses. O grupo total apresentou sobrevida pós-recidiva de 71% em 12 meses e 52% em cinco anos e, entre as 30 mulheres com resposta completa, a SLD foi de 90% em 12 meses e 42% em cinco anos. Apenas o número maior de agulhas esteve associado com pior sobrevida (Razão de Risco = 3,67, p=0,006). A utilização de ultrassonografia como guia de inserção de agulhas não esteve associada com o controle da doença ou a toxicidade. Das mulheres estudadas, 23 (51%) apresentaram toxicidade, sendo 14 casos de fístulas, sem relação com a resposta clínica, se completa ou não, embora tenham ocorrido mais em mulheres que utilizaram quimioterapia e 12 ou mais agulhas para a braquiterapia. Conclusões: A reirradiação com HDR-IB para recidiva pélvica apresentou resposta clínica completa em dois terços das mulheres, com maior sobrevida pós-recidiva e SLD em relação ao esperado para esses casos, embora associada com elevada toxicidade. Apesar da toxicidade observada, esta técnica pode ser uma alternativa para casos selecionados.Palavras-chave: neoplasia do colo do útero; recidiva; braquiterapia; sobrevida.
