Durante a pandemia de Covid-19, a telemedicina foi impulsionada como uma opção que respeita o isolamento social. Dessa forma, o objetivo do presente trabalho foi analisar os fatores que impedem o exercício efetivo da telemedicina no Brasil. Trata-se de uma revisão integrativa da literatura encontrada nas bases de dados Scielo e PubMed a partir com os descritores “telemedicina’’, “Covid-19’’ e “eficácia’’ publicados entre outubro a março de 2021. Os artigos foram selecionados para atender a pergunta científica obtida a partir da estratégia PICO com posterior categorização dos artigos pela Agency for Healthcare Research and Quality. Foram contemplados 44 artigos, dos quais 25 foram excluídos e 19 foram incluídos a partir da análise dos critérios de exclusão e de inclusão. Dessa amostra final, 73,68% encontram-se em nível 6 de evidência, enquanto 31,57% provinham da Espanha e 10,52%, do Brasil. A falta de recursos, infraestrutura e preparação figurou como principal limitante em 57,89% dos artigos. A telemedicina mostra-se uma ferramenta competente no contexto pandêmico e salutar para o pós-pandêmico. Entretanto, ainda são necessários investimentos em recursos tecnológicos e humanos para atingir sua total eficácia.
