Introdução: Os núcleos de preenchimento associados aos pinos de fibra de vidro surgiram para atender à demanda estética e proporcionar melhor distribuição de cargas mastigatórias na raiz, osso e periodonto, minimizando o índice de fraturas radiculares. Objetivo: O objetivo do presente trabalho foi apresentar um relato de caso clínico de reabilitação de dente tratado endodonticamente, utilizando-se um retentor intrarradicular direto confeccionado com pino de fibra de vidro (PFV) individualizado anatomicamente com resina composta e, subsequentemente, coroa metalocerâmica. O presente caso clínico descreve a reabilitação do elemento #11, que apresentava um conduto radicular com diâmetro e forma não compatíveis com os PFV pré-fabricados. Conclusão: Com base no acompanhamento de 5 anos após a instalação, pode-se concluir que tanto os profissionais envolvidos quanto a paciente consideraram satisfatório o resultado clínico. A técnica empregada minimiza a camada de cimento resinoso, que pode causar falhas de retenção do PFV, tendo, portanto, características de adaptação semelhantes à técnica convencional de núcleos metálicos fundidos. Além disso, como visto na literatura, retentores intrarradiculares confeccionados por essa técnica apresentam um módulo de elasticidade bem próximo da dentina remanescente, podendo, assim, evitar fraturas do remanescente dentário com o passar do tempo.
