Resumo Introdução: Reações de hipersensibilidade à droga afetam 7% da população em geral, enquanto a hipersensibilidade a múltiplas drogas é um evento mais raro, com antibióticos sendo agentes de importância nas duas situações. Entretanto, percebe-se que, principalmente na atenção primária, confiar apenas nos relatos dos pacientes para considerá-los alérgicos a antibióticos, sem investigação adequada, pode gerar diagnósticos indevidos. Objetivos: descrever caso de paciente com diagnóstico prévio de hipersensibilidade a múltiplas drogas, proporcionando uma visão crítica sobre a classificação errônea das reações a drogas e suas consequências individuais e na saúde pública. Métodos: forneceu-se TCLE à paciente; após, coletaram-se dados de seu prontuário, relatando caso real atendido no HU-UFSC. Caso clínico: paciente feminina, 38 anos, queixa-se de alergia a múltiplos antibióticos, sem investigação adequada anterior, com necessidade atual de tratamento para infecção urinária. Encaminhada para consulta no NARTAD, onde recebeu administração supervisionada de sulfametoxazol com trimetoprim, sem manifestação de reações posteriores, liberando a medicação para uso pela paciente. Discussão: relatos de alergia a medicamentos, especialmente a antibióticos, devem ser investigados através de anamnese, exame físico e testes alérgicos complementares, quando indicado, além do registro detalhado das informações. A perpetuação de rótulos de alergia a antibióticos, além de efeitos psicológicos nos pacientes, gera custos ao governo com medicações, infecções de sítio cirúrgico, falha terapêutica e iatrogenia. Conclusões: médicos, especialmente da atenção primária à saúde, devem qualificar-se para reconhecer reações de hipersensibilidade a drogas, orientar e encaminhar adequadamente os pacientes, evitando a continuidade dos falsos rótulos alérgicos.
