Background: Glucagon-like peptide 1 agonists differ in chemical structure, duration of action and in their effects on clinical outcomes. The cardiovascular effects of once-weekly albiglutide in type 2 diabetes are unknown. Methods: We randomly assigned patients with type 2 diabetes and cardiovascular disease to the addition of once-weekly subcutaneous injection of albiglutide (30 mg to 50 mg) or matching placebo to standard care. We hypothesized that albiglutide would be noninferior to placebo for the primary outcome of first occurrence of cardiovascular death, myocardial infarction, or stroke. If noninferiority was confirmed by an upper limit of the 95% confidence interval for the hazard ratio of less than 1.30, closed-testing for superiority was prespecified. Findings: Overall, 9463 participants were followed for a median of 1.6 years. The primary composite outcome occurred in 338 of 4731 patients (7.1%; 4.6 events per 100 person-years) in the albiglutide group and in 428 of 4732 patients (9.0%; 5.9 events per 100 person-years) in the placebo group (hazard ratio, 0.78; 95% confidence interval [CI ], 0.68 to 0.90), indicating that albiglutide, was superior to placebo (P<0.0001 for noninferiority, P=0.0006 for superiority). The incidence of acute pancreatitis (albiglutide 10 patients and placebo 7 patients), pancreatic cancer (6 and 5), medullary thyroid carcinoma (0 and 0), and other serious adverse events did not differ significantly between the two groups. Interpretation: In patients with type 2 diabetes and cardiovascular disease, albiglutide was superior to placebo with respect to major adverse cardiovascular events. Introduction: The EDITION development program confirmed that insulin glargine 300 U/mL (Gla-300) provides comparable glycemic control to insulin glargine 100 U/mL (Gla-100) but with lower hypoglycemia risk. Our study aimed to evaluate the effectiveness of Gla-300 in everyday practice. Methods: This one-arm, non-interventional study included patients with type 2 diabetes who were switched to Gla-300-based basal-bolus therapy (BBT) and followed for 6 months. Indications for switching included inadequate glycemic control and/or hypoglycemic events with the previous regimen. Results: Overall 229 patients were included, with mean age of 60.9 years. All glycemic variables improved between baseline and 6 months significantly (mean ± standard deviation [SD] hemoglobin A1c [HbA1c] from 8.9 ± 1.5% to 7.5 ± 1.1%, fasting blood glucose from 9.5 ± 3.1 mmol/L to 7.0 ± 2.1 mmol/L, postprandial blood glucose from 12.0 ± 3.8 mmol/L to 8.9 ± 2.5 mmol/L). Gla-300 doses were increased and mealtime insulin doses were unchanged. Rates of both non-severe and severe hypoglycemic events decreased significantly compared to pre-study and 6-month follow-up periods. Patients switched because of elevated HbA1c had higher baseline HbA1c and greater decrease in HbA1c paralleled with increase in insulin doses compared to those switched because of hypoglycemia. Conclusions: In day-today practice, switching from human insulin to Gla-300-based BBT resulted in significant improvement in glycemic control and decrease in hypoglycemia risk.
A glargin inzulin 300 E/ml és a glulizin inzulin alkalmazása 2-es típusú diabetes mellitusban-hatékonyság a klinikai gyakorlatban Hidvégi Tibor dr.,(1) Balogh Zoltán dr.,(2) Vass Viktor dr.,(3) Kovács Gábor dr.,(4) Stella Péter dr.(5) Összefoglalás Az EDITION program randomizált, kontrollált vizsgálatai igazolták a 300 E/ml glargin inzulinnak (Gla-300) a glykaemiás kontrollra kifejtett eredményességét és a 100 E/ml kiszerelésénél kisebb hypoglykaemia-kockázatát. A jelen, beavatkozással nem járó vizsgálatban a cél a Gla-300 és a napi legalább háromszor adott glulizin inzulin kezelésnek az anyagcserére és a hypoglykaemiás eseményekre kifejtett kedvező hatásának igazolása volt. Módszertan: Az egykaros, 6 hónapos, multicentrikus vizsgálatba humán bázis-bólus kezelésről újonnan analóg inzulinokra váltott 18-80 év közötti betegeket választottak be az érvényes magyar finanszírozási protokollnak megfelelően. A betegeknek 3 és 6 hónappal később volt utánkövetési vizitje, ezeken a glykaemiás változókra, az inzulindózisokra és a hypoglykaemiás eseményekre vonatkozóan gyűjtöttek adatokat. Eredmények: A 229 beteg közül 217 beteg adatait lehetett a biztonságossági, míg 189 betegét a hatásossági elemzésben felhasználni. A betegek vizsgált glykaemiás változói szignifikáns mértékben csökkentek (a HbA 1c 8,9±1,5%-ról 7,5±1,1%-ra, az éhomi vércukorérték 9,5±3,1 mmol/l-ről 7,0±2,1 mmol/l-re, a postprandialis 12,0±3,8 mmol-/l-ről 8,9±2,5 mmol/l-re; mindhárom esetben p<0,001). Folyamatosan nőtt a 8% alatti HbA 1c-célértéket elérő betegek aránya (55,9%, majd 77,8%), az egyéni HbA 1c-célértéket azonban csak a betegek kevesebb mint fele érte el. A beválasztást megelőző 3 hónaphoz képest szignifikánsan csökkent a nem súlyos hypoglykaemiás események incidenciája (9,76/betegév értékről 2,30/betegév értékre). Az incidencia csökkenése különösen az előzetes hypoglykaemiás események miatt átváltottak alcsoportjában volt jelentős. Megbeszélés: A Gla-300 + glulizin inzulin kezelés a vizsgált glykaemiás változóban szignifikáns csökkenést tudott elérni a vizsgálat végéig. Az egyéni HbA 1c-célérték alacsony értéke arra utal, hogy a kezelőorvosok ambiciózus célt állítottak betegeik elé.
