A maior parte dos casos de trauma dentário ocorre no período da infância, principalmente por quedas e acidentes na escola ou em casa. A região da boca mais acometida pelos traumas é a maxila, sendo a mandíbula menos afetada. Uma lesão traumática no dente ocasiona danos a muitas estruturas dentárias e tecidos adjacentes e a possibilidade de êxito do tratamento endodôntico é muito grande quando realizado de maneira correta. O paciente deste caso sofreu um trauma com 11 anos e, mais de uma década depois, com o escurecimento do dente 11, buscou tratamento odontológico e descobriu extensas lesões periapicais envolvendo os dentes 11, 12, 21 e 22. Os dentes 11 e 12 passaram por cirurgia parendodôntica e os dentes 21 e 22 foram submetidos a um tratamento não cirúrgico. Este artigo tem como objetivo comparar a cicatrização de dentes traumatizados com extensas lesões periapicais que passaram por tratamento endodôntico convencional com e sem complementação cirúrgica, além de destacar o uso de materiais biocerâmicos. Concluiu-se que os tratamentos endodônticos com e sem complementação cirúrgica foram efetivos, cada um adequado para as condições individuais de cada elemento dentário.
