In neonates with LONS, sTREM-1 has the potential to provide an excellent predictive value for septic shock/death. Larger sample sizes are needed to identify the optimal cut-off value of plasma sTREM-1 for this diagnosis and to provide diagnostic accuracy measures.
After the perturbing effects of the COVID-19 Pandemic, we observed intense public health efforts in a global-scale to prevent, control, and treat the SARS-CoV-2. Hundreds of clinical center researched for adequate treatments, other were devoted to the development the COVID-19 vaccines and other studied the nature and the effects of this mutant coronavirus SARS-CoV-2. As time goes by, the pediatric inflammatory multisystem syndrome has been recently defined and associated the COVID-19 past infection and is now considered a shot to middle-term complication of COVID-19. In this paper, we review the actual concepts of PIMS in children, the epidemiology, the clinical presentation, and evolution, the recommended laboratory and other testing. The recommended specialties interconsultation for hospitalized patients and a full discussion on the appropriate treatment of these patients. We include the Latin-American experience with PIMS and a final discussion on the outcome of this disease.
Buffy coat culture is as good as TBC for the microbiological diagnosis of late-onset sepsis of the newborn. Buffy coat culture allows the use of remaining plasma for further analysis.
RESUMENIntroducción: el recién nacido (RN) con fiebre es un motivo de consulta frecuente en los servicios de urgencias, y una de las causas es la infección bacteriana grave (IBG). Los objetivos de este estudio fueron establecer la prevalencia de IBG, así como describir las características clínicas, de laboratorio y factores de riesgo en el RN con fiebre, atendidos en urgencias de un hospital ubicado en Monterrey, Nuevo León, México. Material y métodos: estudio observacional, prospectivo y comparativo, realizado entre 2015 y 2016. A cada RN se les realizó un protocolo de estudio que consistió en la elaboración de historia clínica, examen físico y toma de estudios de laboratorio. Los análisis estadísticos realizados fueron con χ 2 , U de Mann-Whitney y análisis de regresión logística. Resultados: se incluyeron 112 neonatos, de los cuales en 28 (25%) se confirmó IBG por la presencia de cultivos positivos en sangre, orina o líquido cefalorraquídeo (LCR). En 17 pacientes (60.7%) se diagnosticó infección de vías urinarias (IVU), en 11 (39.2%) sepsis y en dos (7.1%) meningitis (en ambos, también se detectó bacteriemia). Las bacterias más frecuentes en urocultivo fueron Escherichia coli y Klebsiella pneumoniae; en hemocultivos fue Staphylococcus coagulasa-negativa; mientras que en LCR
