Antecedentes: El tratamiento del síndrome de vejiga hiperactiva incluye desde modificaciones del estilo de vida, fisioterapia, anticolinérgicos, neuromodulacion y toxina botulínica. Debido a la refractariedad ha cobrado interés la neuromodulacion periférica. El objetivo fue evaluar la eficacia de la neuromodulacion transcutánea vs percutánea del nervio tibial en el síndrome de vejiga hiperactiva idiopática no neurogénica. Material y Métodos: Ensayo clínico aleatorizado en 61 pacientes durante enero-octubre de 2019. Se dividió en dos grupos: transcutánea (n=33) o percutánea (n=28). Se uso el estimulador (BioTENS) con 10 Hz, 200 us, durante 12 sesiones, con duración de 20 minutos cada sesión. Se valoro la eficacia mediante el diario vesical de 3 días, cuestionario de síntomas y calidad de vida, y escala de beneficio de tratamiento en la sesión 1, 6 y 12. Se realizo estadística descriptiva y correlación de variables con programa SPSS v22.0, y p ≤ 0.05 estadísticamente significativo. Resultados: La edad media fue 66 años. La frecuencia urinaria diurna y nocturna disminuyo, aumento el volumen urinario promedio, disminuyo el número de episodios de urgencia e incontinencia de urgencia sin diferencias estadísticamente significativa al comparar ambos tratamientos. La percepción de calidad de vida y mejoría en relación al tratamiento mejoro con ambas técnicas (p=>0.05). No hubo efectos secundarios y la adherencia al tratamiento fue del 83.6 % (n=51). Se uso menor intensidad en la percutánea (4 mA vs 7 mA; p=0.0001). Conclusiones: Primer ensayo clínico aleatorizado realizado en el país que evalúa a eficacia en ambas técnicas, demostrando la no inferioridad.

Antecedentes: La colocación de cintas medio uretral son actualmente el tratamiento de primera línea para la incontinencia urinaria de esfuerzo. El objetivo fue evaluar las complicaciones intra y posoperatorias de las cintas transobturadora y retropúbica; y los factores de riesgo asociados. Material y métodos: Estudio de cohorte retrospectivo en 105 mujeres durante el 2016 a 2018. Se dividió en dos grupos: TVT (n=28) y TOT (n=77). Se analizaron variables demográficas, factores de riesgo, complicaciones intra y posoperatorias. Se realizo estadística descriptiva y correlación de variables con programa SPSS v17.0, y p ≤ 0.05 estadísticamente significativo. Resultados: La edad promedio fue 56.7 ± 8.8 años; 31.4% (n=33) con incontinencia de esfuerzo, 65.7% (n=69) con incontinencia mixta (IUM) y 2.9% (n=3) con incontinencia oculta. La tasa de complicación global fue de 29.5% (n=31). La complicación intraoperatoria común fue la perforación vesical 3.8% en el grupo TVT asociada a obesidad (OR 12.9). Las complicaciones posoperatorias estuvieron relacionadas con la TOT (p=0.049), compuestas por infección de vías urinarias (11.4%), inguinodinea (7.6%) y otras (6.9%). El riesgo de infección urinaria posoperatorio estuvo asociado con incontinencia de esfuerzo (p=0.03), ausencia de tratamiento de incontinencia urinaria mixta (p=0.04); aumentado la tasa de incontinencia urinaria de urgencia de novo. Conclusiones: El enfoque transobturador tiene bajos índices de complicaciones transoperatorias en comparación con la retropúbica. La complicación más común en nuestra serie fue la infección urinaria por lo que se requieren estudios prospectivos aleatorizados.

Antecedentes: En las últimas décadas, el manejo postquimioterapia de seminoma ha evolucionado debido al uso creciente de tomografía por emisión de positrones con 18 fluoro-desoxi-D-glucosa (18-FDG-PET) este método solo o combinado con la tomografía computarizada (CT) se ha propuesto como herramienta no invasiva para evaluar la extensión de la enfermedad. El objetivo fue conocer su utilidad para determinar viabilidad tumoral en el manejo de masas residuales retroperitoneales de seminoma postquimioterapia. Material y Métodos: Estudio retrospectivo monocéntrico, se revisaron expedientes clínicos de 53 pacientes durante 2013-2018. Se definió respuesta a tratamiento mediante criterios de Recist 1.1 por CT y viabilidad tumoral mediante Suv Max por 18-FDG-PET/CT. Según resultado se dividió en grupos: vigilancia (grupo 1), cirugía de rescate (grupo 2), radioterapia (grupo 3) y quimioterapia (grupo 4). Se realizo estadística descriptiva con programa SPSS v24. Se uso curva ROC para definir punto de corte, sensibilidad (S) y especificidad (E). Resultados: Al analizar, sin discriminar por grupos, se encontró una curva ROC con punto de corte de 3.150, S 50% y E 60%, valores que no son discriminativos ni útiles para definir viabilidad tumoral. Al analizar el grupo de vigilancia y cirugía de rescate se obtuvieron S 80% y E 82%. Al comparar con los grupos 3 y 4 tuvieron un punto de corte de 3.150 y 2.350 respectivamente. Conclusiones: Primer estudio realizado en el país, el 18 FDG-PET/CT demostró una utilidad muy pobre para discriminar viabilidad tumoral en masas residuales seminomatosas postquimioterapia, mejorando discretamente los resultados para los grupos de vigilancia y cirugía de rescate.
