Разработка молекулярно-направленной антицитокиновой терапии стала прорывом в лечении многих аутовоспалительных заболеваний и прежде всего ревматоидного артрита (РА). При-менение моноклональных антител и ингибиторов малых моле-кул, направленных против провоспалительных цитокинов, их рецепторов и сигнальных путей, позволило контролировать ос-новные клинические проявления аутоиммунных воспалитель-ных заболеваний. Один из следующих этапов в изучении эффек-тов генно-инженерных биологических препаратов (ГИБП) по-священ оценке генерализованной потери костной ткани и пред-упреждению переломов костей.РА -аутоиммунное ревматическое заболевание (РЗ) неиз-вестной этиологии, характеризующееся хроническим эрозивным артритом (синовитом) и системным воспалительным поражени-ем внутренних органов [1]. Известно, что при РА, как и при мно- АннотацияРиск переломов у больных хроническими воспалительными заболеваниями, в том числе ревматическими, превышает таковой в популяции. Одним из наиболее обсуждаемых и патогенетически значимых факторов снижения минеральной плотности костной ткани (МПКТ), нарушения ее качества и повышения риска переломов является хроническое воспаление. Хронический воспалительный процесс способствует дисбалансу ремоделирования кости в сторону резорбции костной ткани. Применение генно-инженерных противовоспалительных препаратов (ГИБП) у пациентов с ревматическими и другими аутоиммунными заболеваниями приводит к эффективному подавлению системного воспаления, повышению МПКТ. Chronic inflammation is one of the most discussed and most important pathogenic factors of reduced bone mineral density (BMD), its worse quality and increased fracture risk. The chronic inflammatory process contributes to the imbalance of bone remodeling toward bone resorption. The use of biological agents in patients with rheumatic and other autoimmune diseases effectively suppresses systemic inflammation and increases BMD. However, the effect of the biological agents in lessening the risk of fractures remains unproven.
