Background: Dynamic contrast-enhanced magnetic resonance imaging is a noninvasive imaging modality that can supply information regarding the tumor anatomy and physiology. The aim of the study was to analyze DCE-MRI perfusion parameters in normal pancreatic parenchymal tissue and PDAC and to evaluate the efficacy of this diagnostic modality in determining the tumor grade. Methods: A single-center retrospective study was performed. A total of 28 patients with histologically proven PDAC underwent DCE-MRI; the control group enrolled 14 patients with normal pancreatic parenchymal tissue; the radiological findings were compared with histopathological data. The study patients were further grouped according to the differentiation grade (G value): well- and moderately differentiated and poorly differentiated PDAC. Results: The median values of Ktrans, kep and iAUC were calculated lower in PDAC compared with the normal pancreatic parenchymal tissue (p < 0.05). The mean value of Ve was higher in PDAC, compared with the normal pancreatic tissue (p < 0.05). Ktrans, kep and iAUC were lower in poorly differentiated PDAC, whereas Ve showed no differences between groups. Conclusions: Ve and iAUC DCE-MRI perfusion parameters are important as independent diagnostic criteria predicting the probability of PDAC; the Ktrans and iAUC DCE-MRI perfusion parameters may serve as effective independent prognosticators preoperatively identifying poorly differentiated PDAC.
Polycystic ovary syndrome (PCOS) is a common endocrinopathy among women of reproductive age associated with hyperandrogenism, oligo-amenorrhea, and infertility. Symptoms and their severity vary among the individuals. If the manifestation is mild, PCOS may remain undiagnosed. In more severe cases, it results in a spectrum of symptoms of metabolic syndrome, insulin resistance, and cardiovascular diseases. The diagnosis is established after a physical examination and evaluating the patient’s hormonal profile. In addition to these required methods, ultrasonographic assessment of the patient’s ovaries is another non-invasive, cheap, and time-saving tool, making the examination more profound and leading to the correct diagnosis. Specific ultrasonographic parameters are used to tell the healthy and polycystic ovaries apart: the ovarian volume (OV), ovarian follicle count, follicle distribution pattern, ovarian stromal echogenicity, and the resistance and pulsatility indices assessed using the Doppler function. This review evaluated the selected articles and ascertained the ultrasonographic parameters that accurately predict PCOS. This systematic review showed that the most valuable ultrasonographic parameters in diagnosing PCOS are the OV and follicle number per ovary.
for CD. 3 Magnetic resonance enterocolonography (MR -EC) is a noninvasive imaging modality that complements ileocolonoscopy. The MR -EC advantages are the absence of ionizing radiation, high soft -tissue resolution, and unique diffusion--weighted imaging (DWI) sequence. The DWI creates contrast through the motion of water and other small molecules within tissue, 4 which allows qualitative and quantitative functional analysis. The sequence is not time consuming, and the most significant advantage is that no intravenous contrast is needed. 4 Different pathological features, such as cell density, extracellular matrix, nucleic areas, and membrane permeability, IntroductIon Crohn disease (CD) usually affects young adults and can have a progressive and disabling course. Therefore, early diagnosis and monitoring of activity are essential. Endoscopic and histopathological evaluation of CD activity remains a reference standard. 1 Therefore, the usefulness of inflammatory markers is increasingly being explored, and the results are promising. Raktažodžiai: magnetinio rezonanso enterokolonografija, uždegiminės žarnyno ligos, uždegimo aktyvumas. SantraukaMagnetinio rezonanso tomografijos (MRT) enterokolonografija, tai -tyrimas, kurio metu galime ištir-ti visą plonąjį ir storąjį žarnyną. Didžiausias tyrimo privalumas, kad galima įvertinti ne tik žarnos spindį, sieneles, pakitimus už žarnos sienelės ribų, bet ir diagnozuoti aktyvią uždegiminę žarnyno ligą, atskirti Krono ligą nuo opinio kolito ir nustatyti Kronos ligos aktyvumo laipsnį. Darbo tikslas-įvertinti MRT enetrokolonografijos galimybę nustatyti uždegiminės žarnyno ligos požymius ir šių parametrų panaudojimą uždegiminių žarnyno ligų diagnostikoje. Metodika. Trisdešimt pacientų kliniškai įtarus aktyvią uždegiminę žarnyno ligą įtraukti į tyrimą. MRT enterokolonografijos tyrimas atliktas kaip šio klinikinio tyrimo dalis. Pagal gautus tyrimo rezultatus pacientai buvo suskirstyti į dvi grupes: kontrolinę ir sergančiųjų aktyvia uždegimine žarnyno liga. Visas žarnynas buvo padalintas į devynis segmentus. Kiekvienas segmentas buvo vertinamas dėl požymių, būdingų uždegiminei žarnyno ligai. Rezultatai. Nustatyta ženkli teigiama koreliacija tarp klinikinių ligos aktyvumo indeksų ir sienelės kontrastinės medžiagos kaupimo savybių, haustrų nebuvimo (atitinkamai r= 0,512, r=0,585, p=0,01), T2 žarnos sienelės hiperintensinio signalo ir reliatyvaus kontrastinės medžiagos kaupimo (r= 0,785, p=0,01). Vidutiniška koreliacija buvo nustatyta tarp klinikinių ligos aktyvumo indeksų ir sienelės storio, opų, reliatyvaus kontrasto kaupimo (atitinkamai r=0,379, r=0,384, r=0,397, p=0,05 ĮžangaMagnetinio rezonanso tomografijos tyrimas diagnozuojant uždegimines žarnyno ligas (UŽL) tampa vis svarbesnis. MRT tyrimas pasižymi tokiomis savybėmis: gera skiriamąja minkštųjų audinių raiška, statiniais ir dinaminiais vaizdais, galimybe gauti vaizdus įvairiose plokštumose [1].MRT yra populiarus tyrimas dėl to, kad neturi jonizuojančios spinduliuotės poveikio, skaitmeniniai tyrimo vaizdai gali būti archyvuojami, tai suteikia galimybę pirminius vaizdus palyginti su vėlesniais, atliekamais gydymo eigoje.Opinis kolitas (OK) ir Krono liga (KL) yra neaiškios etiologijos, lėtinės, pasikartojančios eigos uždegiminės žarnyno ligos [2,3].MRT enterokolonografija, tai tyrimas, kurio metu galime ištirti visą plonąjį ir storąjį žarnyną. Didžiausias tyrimo privalumas, kad galima įvertinti ne tik žarnos spindį, sieneles, pakitimus už žarnos sienelės ribų, bet ir diagnozuoti aktyvią UŽL, atskirti KL nuo OK ir nustatyti Kronos ligos aktyvumo laipsnį [4].MRT jautrumas ir specifiškumas diagnozuojant KL atitinkamai yra 78% (95% CI 67-84%) ir 85% (95% CI 76-90%) [5].UŽL aktyvumui, išplitimui ir sunkumui įvertinti yra ypač svarbu pasirinkti tinkamus diagnostikos metodus. Dažniausiai tenka pasikliauti endoskopiniu tyrimu. Vis dėlto kolonoskopija yra invazinis tyrimas, sukeliantis diskomfortą, ypač kai liga ūmi ir sunki, taip pat išlieka žar-nos perforacijos rizika [2].
