Mechanisms of the development of pain in chronic venous diseases, including pelvic congestion syndrome, are not studied in detail so far. The existing hypotheses of the occurrence of venous pelvic pain do not allow to answer the question why some patients have no pain syndrome, while others have very pronounced pain despite the same morphofunctional changes in the pelvic veins. This review presents current hypotheses of the venous pelvic pain development, data on some vasoactive neuropeptides (endothelin, calcitonin gene-related peptide, and substance P), their role in the modulation of vascular tone and sensation of pain, and possible association between neurogenic inflammation and venous pelvic pain, as well as provides rationale for studying the activity of these neurotransmitters in the treatment of pelvic congestion syndrome and pelvic pain.
The review deals with the problem of venous trophic ulcers. Current data on epidemiology and social significance of this condition are presented with special reference to pathogenetic aspects of its development, such as systemic immunity disorders associated with chronic venous insufficiency. The necessity and possibility of correction of some of them are discussed.
Количество пациентов с венозными трофиче-скими язвами (ВТЯ) неуклонно растет. По данным зарубежных авторов [1], заболеваемость ВТЯ со-ставляет 0,20-0,35% в год. Примерно 25% населе-ния западных стран и 40% населения Российской Федерации страдают заболеваниями вен [2], в том Цель исследования -оценка эффективности профилактики рецидива венозных трофических язв специальным ком-плектом гольфов. Материал и методы. Обследованы 20 пациентов в возрасте 40-70 лет с зажившей венозной язвой (С5 по СЕАР). Для профилактики рецидива язв использовали компрессионные гольфы в течение 12 мес. Проводили оценку клинических проявлений хронической венозной недостаточности (ХВН) и изменений в микроциркуляторном русле с помощью капилляроскопии. Результаты. Давление, оказываемое комплектом гольфов, сохранялось около 40 мм рт.ст. в течение всего времени наблюдения. Клинические проявления ХВН, такие как отек, уплотнение и гиперемия кожи к концу исследования значительно уменьшились. Изменения окружности голени были наиболее выражены в средней трети: до лечения 42,05±3,67 см, после лечения 40,78±3,56 см (p=0,05). Наряду с этим положительные результаты зарегистриро-ваны в изменении окружности голени в нижней трети: от 25,97±3,42 до 24,96±3,52 см (p=0,05). Также выявлены измене-ния, свидетельствующие об улучшении микроциркуляции: уменьшение количества капилляров 2-3-го типа и появление «классических» неизмененных капилляров 1-го типа. Наряду с этим увеличилась скорость капиллярного кровотока, изме-нилось соотношение диаметров капилляров -превалировало увеличение переходных отделов, периваскулярной зоны. Заключение. Адекватное компрессионное лечение оказывает положительное влияние на регресс клинических симптомов ХВН нижних конечностей, улучшает микроциркуляцию и является свидетельством необходимости регулярного примене-ния эластической компрессии нижних конечностей. Ключевые слова: хроническая венозная недостаточность, венозные трофические язвы, эластичная компрессия, медицинский компрессионный трикотаж, компьютерная капилляроскопия.Aim -the objective if the present study was to estimate the effectiveness of the prevention of recurrent trophic venous ulcers with the use of special knee-high compression stockings. Material and methods. The study included 20 patients at the age of 40-70 years presenting with the healed trophic ulcers (C5 according to CEAP). Wearing special knee-high compression stockings was prescribed to prevent the relapse of ulceration during 12 months. The main characteristics recorded throughout the study period included the clinical manifestations of chronic venous insufficiency (CVI) and the changes in the microcirculation system. Results. The pressure of the knee-high stockings (around 40 mm Hg) persisted throughout the observation period. The clinical manifestations of chronic venous insufficiency, such as oedema, skin induration and hyperemia significantly decreased by the end of the study.
