RESUMO: Os nervos periféricos sofrem constantes lesões de origem traumática, o que resulta em perdas funcionais. A terapia com laser de baixa potência vem sendo utilizada para minimizar os efeitos maléficos da inflamação e acelerar o processo de cicatrização dos tecidos lesados. Este estudo teve como objetivo verificar o efeito da irradiação do laser 830 nm no comportamento do nervo isquiático de ratos submetido a esmagamento. Foram utilizados 20 ratos, todos tendo tido o nervo isquiático esmagado, divididos em 4 grupos (n=5): P7 e P14, tratamento placebo por 7 e 14 dias; L7 e L14, tratamento por laser (dosagem de 4 J/cm²) por 7 e 14 dias. Os animais dos grupos P7 e P14 foram submetidos aos mesmos procedimentos, mas com o laser desligado. Os parâmetros analisados foram presença de infiltrado inflamatório e fibroblastos, destruição da bainha de mielina e degeneração axonal. Na análise estatística foi observada diferença estatística com relação a três parâmetros: os animais do grupo L14 apresentaram maior quantidade de fibroblastos (p=0,0001), menor degeneração da bainha de mielina (p=0,007) e menor quantidade de infiltrado inflamatório (p=0,001). A aplicação do laser de baixa potência contribuiu para a redução do processo inflamatório decorrente da lesão do nervo isquiático de ratos. Low-power laser therapy has been used in order to minimize harmful effects of inflammation and to accelerate healing of injured tissues. The purpose of this study was to assess the effect of 830 nm-laser irradiation on rat sciatic nerves submitted to crush. Twenty male Wistar rats had their sciatic nerve crushed and were divided into 4 groups (n=5): Sham7 and Sham14, placebo-treated for 7 and 14 days; L7 and L14, laser-treated (at 4 J/cm²) for 7 and 14 days. Sham group animals were submitted to the same procedures, but with the laser turned off. Assessed parameters were inflammatory infiltrates, fibroblasts, myelin sheath destruction, and axonal degeneration. The statistical analysis showed significant differences in three parameters: L14 animals showed more fibroblasts (p=0.0001), lesser myelin sheath degeneration (p=0.007), and lesser inflammatory infiltrates (p=0.001). Low-power laser therapy hence contributed to reduce the inflammatory process due to rat sciatic nerve injury.
SUMMMARYThe interpersonal reproducibility of the method for evaluating the Sciatic Functional Index (SFI) was measured by a computer-based program developed for this purpose. Twenty Wistar rats were used, and their right sciatic nerve was addressed using general anesthesia. Those nerves were subsequently smashed in a segment of 5 mm proximal to its trifurcation with a special device, with a fixed load of 5 kg for 10 minutes. Animals' footprints were taken at preoperative phase and then in a weekly basis, from the 1st to the 8th week, in a gait track. Prints were digitalized, stored and assessed by predetermined parameter measures, by four examiners, always following the same sequence of parameters marking. Results were submitted to statistical analysis, which evidenced the existence of a high correlation among examiners at preoperative evaluation and on the 3rd, 4 th , 5 th , 7 th , and 8 th weeks (equal or higher than 0.82), with an eventual drop on 6 th week, but still as significant as the others (pF<0.01). On the 1st and 2nd weeks, the correlation index was nearly zero, showing the low reproducibility of the method in that period, in which variation among animals did not differ from the variation among examiners (p=0.24 and 0.32, respectively), due to the poor sharpness of footprints.
RESUMOIntrodução: Os efeitos da irradiação ultra-sônica terapêutica na regeneração de nervos periféricos não são ainda bem conhecidos, particularmente no que se refere à recuperação funcional. Por outro lado, o método da avaliação da impressão da pegada do rato para medida do Índice Funcional do Ciático (IFC) já está bem sistematizado, mostrando estreita correlação com a regeneração morfológica do nervo ciático lesado em ratos. Objetivos: Analisar a influência do ultra-som terapêutico na regeneração do nervo ciático de ratos submetido a esmagamento controlado. Material e métodos: Foram empregados 20 ratos da linhagem Wistar com peso corporal médio de 300 g, divididos em dois grupos conforme o tipo de procedimento realizado: 1) somente esmagamento (n=10); 2) esmagamento e irradiação com ultra-som (n=10). Sob anestesia geral, o nervo ciático era exposto na coxa direita do animal e esmagado com um dispositivo especialmente confeccionado para essa finalidade, com uma carga fixa de 15 Kg por 10 minutos, num segmento de 5 mm proximal à sua bifurcação. A irradiação ultra-sônica pulsada de baixa intensidade (1:5, 0,4 W/ cm 2 , 1 MHz, duração 2 minutos) era iniciada já no primeiro dia pós-operatório e realizada por dez dias consecutivos. As impressões das pegadas dos animais, obtidas semanalmente, da primeira à terceira semana pós-operatória, em passarela específica para essa finalidade, foram avaliadas por meio de um programa de computador igualmente específico, segundo método pré-existente já testado em trabalhos anteriores, com cálculo automático do IFC. Resultados: O IFC aumentou progressivamente nos dois grupos, passando no Grupo 2 de -101 na primeira semana, para -59,21 na segunda e -26,68 na terceira, o que significou uma melhora de 73% entre a primeira e a última medida. No Grupo 1, o IFC subiu de -98,2 na primeira semana, para -79,5 na segunda e -44 na terceira, o que significou uma melhora de 55% entre a primeira medida e a última. As diferenças entre os grupos foram significativas no 14 o e no 21 o dia (p=0,02 e p=0,002, respectivamente). Conclusão: O ultra-som terapêutico de baixa intensidade acelerou a regeneração do nervo ciático do rato, demonstrável com maior significância no 21 o dia pós-operatório.Palavras-chave: regeneração de nervo periférico, nervo ciático de ratos, lesão por esmagamento, ultra-som terapêutico, índice funcional do ciático, avaliação funcional. Objective: To mechanically and histologically evaluate the application of low-level laser therapy to the reparative process on lesions caused by impact on the gastrocnemius muscles of rats. Methods: 45 female Wistar rats were divided into three groups (n=15/ group): C (control, no lesion), ML (muscle lesion) and ML-L (muscle lesion and laser therapy). The experimental muscle lesion was produced by letting a 250 g load drop from a height of 30 cm, directly onto the muscle. The animals in the ML-L group were subjected to application of 960 nm laser, 2 J/cm2, on the lesion site, for three days, twice a day. Mechanical tests were performed on an Emic® universal testing machine. Results: The mean values for the maximum force were: 35.70 (± 2.69) N in group C, 31.77 (± 2.59) N in group ML and 34.36 (± 3.63) N in group ML-L, with a statistically significant difference between groups C and ML (p < 0.05). The mean values for relative stiffness were: 3.75 (± 0.98) N/mm in group C, 3.84 (± 0.32) N/mm in group ML and 4.43 (± 0.68) N/mm in group ML-L, with no statistically significant differences (p>0.05). Histological analysis showed the presence of blood vessels in group ML-L and hematomas during the repair process. Conclusion: Laser therapy had a positive effect on the regeneration process of the muscle injury.
REsUMOObjetivo: Avaliar, mecânica e histologicamente, a aplicação do laser terapêutico de baixa potência, no processo reparativo de lesões provocadas por impacto em músculos gastrocnêmios de ratas. Métodos: 45 ratas Wistar foram divididas em três grupos (n=15/grupo): C (controle, sem lesão), LM (lesão muscular) e LM-L (lesão muscular e laserterapia). A lesão muscular experimental foi produzida pela queda de uma carga de 250 g, de uma altura de 30cm, diretamente sobre o músculo. Os animais do grupo LM-L foram submetidos à aplicação de laser 960nm, 2J/cm 2 , durante três dias, duas aplicações por dia, no local da lesão. Foi realizado ensaio mecânico na máquina universal de ensaio Emic ® . Resultados: As médias dos valores de força má-xima foram: (35,70 ± 2,69) N no grupo C, (31,77 ± 2,59) N no grupo LM e (34,36 ± 3,63) N no grupo LM-L, com diferença estatística significativa nos grupos C e LM (p < 0,05). As mé-dias dos valores de rigidez relativa foram (3,75 ± 0,98) N/mm no grupo C, (3,84 ± 0,32) N/mm no grupo LM e (4,43 ± 0,68) N/mm no grupo LM-L, sem diferença estatística (p > 0,05). A análise histológica evidenciou presença de vasos sanguíneos no grupo LM-L e hematomas em processo de reparação.
