Mục tiêu: xác định sự biến đổi của khí máu động mạch và kết quả điều trị của bệnh nhân bỏng hô hấp điều trị tại khoa Hồi sức cấp cứu, Bệnh viện Bỏng Quốc gia Lê Hữu Trác.Đối tượng và phương pháp nghiên cứu: Nghiên cứu tiến cứu trên 60 bệnh nhân bỏng nặng điều trị tại khoa Hồi sức cấp cứu, Bệnh viện Bỏng Quốc gia Lê Hữu Trác từ tháng 05/2021 đến tháng 05/2022. Bệnh nhân được chia làm 2 nhóm tương đồng về tuổi và diện tích bỏng. Nhóm nghiên cứu gồm 30 bệnh nhân được chẩn đoán xác định bỏng hô hấp, nhóm chứng gồm 30 bệnh nhân bỏng nặng không có bỏng hô hấp. Các chỉ tiêu theo dõi đánh giá bao gồm: Khí máu động mạch tại thời điểm nhập viện (cả 2 nhóm) và các ngày tiếp theo (đối với nhóm nghiên cứu), các biến chứng và kết quả điều trị.Kết quả nghiên cứu: Tình trạng nhiễm toan chuyển hóa lúc nhập viện ở bệnh nhân bỏng hô hấp nặng hơn bệnh nhân bỏng nặng không có bỏng hô hấp với biểu hiện BE -9,3 ± 12,2mmol/l so với -1,7 ± 5,7mmol/l (p < 0,05) và lactat máu 4,52 ± 2,52mmol/l so với 2,6 ± 1,65mmol/l (p < 0,05). Trong tuần đầu sau bỏng, các bệnh nhân bỏng hô hấp đáp ứng thở máy tốt với tỷ lệ PaO2/FiO2 thường xuyên trên 250mmHg, nồng độ lactat máu giảm dần về giá trị bình thường. Tỷ lệ tử vong của bệnh nhân bỏng hô hấp cao hơn nhiều so với nhóm bệnh nhân không bỏng hô hấp (60% ở nhóm bệnh nhân bỏng hô hấp so với 3,33% ở nhóm bệnh nhân không bỏng hô hấp với p < 0,001).Kết luận: Bệnh nhân bỏng hô hấp nhiễm toan chuyển hóa nặng thời điểm nhập viện, đáp ứng thở máy tốt trong tuần đầu sau bỏng và tỷ lệ tử vong cao.
