This article discusses the implications of decentralization in the light of international and Brazilian federalism, and its effects on public health policy. In a comparative analysis among countries, the authors find there is no single model; rather, each country has a unique structure of institutions and norms that have important implications for the operation of its health system. Brazil shares some similarities with other countries that have adopted a decentralized system and is assuming features ever closer to U.S. federalism, with a complex web of relationships. The degree of inequality among Brazilian municipalities and states, along with the budgetary imbalances caused by the minimal levels of resource utilization, undermines Brazil's constitutional principles and, consequently, its federalism. To ensure the constitutional mandate in Brazil, it is essential, as in other countries, to create a stable source of funds and increase the volume and efficiency of spending. Also important are investing in the training of managers, improving information systems, strengthening the principles of autonomy and interdependence, and defining patterns of cooperation within the federation.
A atualidade é modelada por um processo de aceleração da mobilidade de pessoas, decorrente do processo de globalização e da consequente produção de expectativas de incremento da qualidade de vida em países de acolhimento. Neste contexto e tendo como objeto de estudo o acesso dos imigrantes à saúde, uma análise comparativa entre Portugal e Brasil, adotamos como metodologia, uma pesquisa qualitativa documental, sobre a discussão da globalização como impulsionadora dos fluxos migratórios, contextualizando a legislação em vigor referente as politicas imigratórias e dos sistemas de saúde, suas características, e o binómio imigrante/ saúde em cada um dos países, além da aplicação de 48 questionários sobre questões socioeconômicas, imigração e acesso à saúde. As conclusões indicam que as Constituições e o aparelho legislativo asseguram, formalmente, o acesso à saúde em ambos os países. Portugal soube desenvolver, ao longo dos anos, políticas públicas de integração de qualidade capazes de garantir boas respostas para as necessidades específicas dos estrangeiros. Porém, apesar de Portugal ter optado por um sistema de saúde universal que abrange a todas as pessoas, e de ter uma normativa que permite o acesso dos imigrantes aos serviços de saúde, ainda persistem desigualdades consideráveis. O Brasil vem buscando implementar o direito à saúde também para os imigrantes mas a legislação não está sendo suficiente para atender às necessidades específicas. É importante destacar que, enquanto o imigrante em Portugal recorre essencialmente ao Serviço Nacional de Saúde (SNS), no Brasil a opção é pela utilização do serviço de saúde privado.
