BackgroundAtrial fibrillation is associated with higher mortality. Identification of causes of death and contemporary risk factors for all‐cause mortality may guide interventions.Methods and ResultsIn the Rivaroxaban Once Daily Oral Direct Factor Xa Inhibition Compared with Vitamin K Antagonism for Prevention of Stroke and Embolism Trial in Atrial Fibrillation (ROCKET AF) study, patients with nonvalvular atrial fibrillation were randomized to rivaroxaban or dose‐adjusted warfarin. Cox proportional hazards regression with backward elimination identified factors at randomization that were independently associated with all‐cause mortality in the 14 171 participants in the intention‐to‐treat population. The median age was 73 years, and the mean CHADS 2 score was 3.5. Over 1.9 years of median follow‐up, 1214 (8.6%) patients died. Kaplan–Meier mortality rates were 4.2% at 1 year and 8.9% at 2 years. The majority of classified deaths (1081) were cardiovascular (72%), whereas only 6% were nonhemorrhagic stroke or systemic embolism. No significant difference in all‐cause mortality was observed between the rivaroxaban and warfarin arms (P=0.15). Heart failure (hazard ratio 1.51, 95% CI 1.33–1.70, P<0.0001) and age ≥75 years (hazard ratio 1.69, 95% CI 1.51–1.90, P<0.0001) were associated with higher all‐cause mortality. Multiple additional characteristics were independently associated with higher mortality, with decreasing creatinine clearance, chronic obstructive pulmonary disease, male sex, peripheral vascular disease, and diabetes being among the most strongly associated (model C‐index 0.677).ConclusionsIn a large population of patients anticoagulated for nonvalvular atrial fibrillation, ≈7 in 10 deaths were cardiovascular, whereas <1 in 10 deaths were caused by nonhemorrhagic stroke or systemic embolism. Optimal prevention and treatment of heart failure, renal impairment, chronic obstructive pulmonary disease, and diabetes may improve survival.Clinical Trial Registration URL: https://www.clinicaltrials.gov/. Х ронічна серцева недостатність (ХСН) характеризуєть-ся високим рівнем смертності та частоти госпіталіза-ції пацієнтів, істотним зниженням якості життя та значним фінансовим тягарем [1]. ХСН -медико-соціальна проблема, котрій прогнозують домінуючі позиції в усьому світі у близькому майбутньому, оскільки середня тривалість життя хворих із моменту встановлення діагнозу становить від 1,7 до 7 років і при тривалому спостереженні свідчить про під-вищення ризику смерті (на 5 рік у чотири рази перевищує початкові значення). Поширеність ХСН серед населення становить 2-3%, а в старшій віковій групі -10%. Витрати на лікування пацієнтів із ХСН вдвічі перевищують кількість коштів, що виділяються на лікування всіх форм раку [2].Основними етіологічними чинниками виникнення, про-гресування та несприятливого виходу ХСН є вік, ішемічна хвороба серця (ІХС), артеріальна гіпертензія (АГ), клапанна хвороба серця, цукровий діабет та ожиріння [3]. Протягом Фремінгемського дослідження доведено, що незалежним фактором серцево-судинної захворюваності та смертності є гіпертрофія лівого шлуночка (ЛШ), яка відіграє важливу роль серед механізмів формування дисфункції міокарда [4]. Вірогідність розвитку серцево-судинної патології в осіб з ожирінням на 50% більша, ніж у осіб із нормальною масою тіла [5]. Встановлено вірогідний зв'язок надмірної ваги та помірного ожиріння зі збільшенням випадків вперше ви-явленої ХСН [6].Структурні зміни серця при ожирінні можна поділити на такі основні складові: гіпертрофія ЛШ, зміни структурної побудови серцевої тканини, ожиріння серця, зміни розмірів правого шлуночка та лівого передсердя (ЛП), клапанна хвороба серця [4].Багато дослідників установили незалежну асоціацію ожиріння з гіпертрофією ЛШ [7]. Деякі вчені вважають, що збільшення маси ЛШ при ожирінні є пропорційним збільшенню площі поверхні тіла та не є патологічним [8]. Попередні дослідження довели, що при ожирінні має місце дилатація камер серця [3]. За наявності ожиріння товщина стінки ЛШ у більшості випадків переважає ступінь ди-латації його порожнини (концентрична гіпертрофія ЛШ (КГЛШ)) [4]. Визначено також більший розмір ЛП у хворих на ожиріння у порівнянні з групою осіб із нормальною вагою. Механізми, що призводять до збільшення розміру ЛП, ідентичні тим, котрі зумовлюють гіпертрофію ЛШ: збільшення індексу маси тіла (ІМТ), гіпертензія, об'ємне перевантаження та порушення діастолічного наповнення. Framingham Heart Study серед хворих на ожиріння показало більший ризик виникнення фібриляції передсердь, що по-яснювалось саме збільшенням розміру ЛП [9]. Обстежили 153 хворих на хронічну серцеву недостатність ІІ функціонального класу, які мали нормальну, надлишкову масу тіла та абдомінальне ожиріння І-ІІІ ступенів. Досліджували структурні зміни міокарда в умовах хронічної серцевої недостатності залежно від ступеня зайвої ваги. In the modern scientific world, it has been proven that non-alcoholic fatty liver disease (NAFLD) is a marker of the risk of cardiovascular (CV) events, and therefore, attention and control of risk factors for CV diseases is important. Considering the prevalence of atherogenic dyslipidemias and their proven effect on the development of thrombotic CV complications in patients with NAFLD, it is important to understand the role of hemostatic blood activity. The objective: To increase the efficiency of early diagnosis of thrombophilic changes in the blood in patients with essential hypertension (HT) combined with non-alcoholic fatty liver disease by determining the state of plasma hemostasis in conditions of hypercholesterolemia and concomitant statin therapy. Materials and methods. 152 patients were examined. Patient groups: I – 46 patients with stage II hypertension, II – 54 patients with NAFLD without hypertension, group III – 52 patients with stage II hypertension with concomitant NAFLD. Results. The growth of prothrombogenic activity of the blood among all groups of patients, however, with HT II stage, combined with NAFLD, the most significant effect was carried out precisely on the final stages of coagulation. An increase in the level of fibrinogen was observed in patients with grade II hypertension. by 29,3 % (p<0,01) and with a combination of HT and NAFLD by 39,7 % (p<0,001). The levels of soluble fibrin-monomeric complexes in all groups were significantly higher than the control values: in patients with hypertension. – 4,1 times (p<0,001), with NAFLD – 2,8 times (p<0,001), in the NAFLD group with hypertension – 4,5 times (p<0,001). Antithrombin III (AT III) was reduced by 12,3 % (p<0,01) relative to the control only in patients with hypertension. On the other hand, the fibrinolytic potential was reduced among all examined groups. Determination of the effect of lipid-lowering therapy revealed an acceleration of prothrombin time (PTT) by 19.2 % (p<0,01) in patients with NAFLD without statin treatment. In the general cohort, statin use increased the activity of AT III by 10,7 % (p<0,01), but in the NAFLD group, this difference was more significant – by 14,3 % (p<0,001). In patients with comorbid course of HT and NAFLD with cholesterolemia level <5 mmol/L, we observed an increase in PTT by 32,5 % (p<0,05), INR by 25,4 % (p<0,05) and thrombin time by 23,2 % (p<0,05) during statin therapy. On the other hand, in the subgroup with hypercholesterolemia, statins increased the activity of the anticoagulant link of hemostasis – the level of ATIII increased by 3,1 % (p<0,05). Conclusions. Depletion of the fibrinolytic potential against the background of activation of the coagulant hemostasis link is observed in patients with hypertension combined with NAFLD. In the case of concomitant hypercholesterolemia, the procoagulant activity of the blood increases, however, against the background of treatment with statins, there is a decrease in the coagulation potential of the blood and an increase in the activity of the anticoagulant link of hemostasis.
Introduction: Obesity due to the high proportion of visceral adipose tissue is often leads to hypertension (HT), non-alcoholic fatty liver disease (NAFLD). The risk of thrombogenic complications increases in the presence of NAFLD and HT. Considering the occurrence of prothrombotic changes in the blood in patients with NAFLD, HT and obesity, the combination of these diseases may be an additional risk of thrombosis. Methods: The research was conducted at the Bogomolets National Medical University, at Department Propaedeutics of Internal Medicine No 1. We examined 152 patients and 15 individuals for control group. Results: Mean platelet count was higher in obese patients by 6.4% (p<0.05). In patients with NAFLD without HT, platelet count was higher in the presence of obesity by 16% (p<0.05), we have seen an increase level of MPV in NAFLD patients with obesity. In the comorbid course of HT and NAFLD, aggregation of platelets stimulated by adrenaline was grown in case of obesity (18%; p<0.001) relatively to non-obese patients. In obese patients suffering from HT, these hemostasis changes were characterized by a tendency to accelerate coagulation. There was a reduction in prothrombin time (PT) time by 8.1%, a growth in fibrinogen by 25.8% (p<0.001) and soluble fibrin monomeric complexes (SFMC) by 4 times (p<0.001) in obesity. Conclusion: Obesity increases the thrombogenic activity of blood in both patients with HT and in patients with HT and NAFLD, both due to platelet and coagulation of hemostasis.
