Purpose — to discover peculiarities of Wilson disease course in children dependently on the variant of liver affection. Materials and methods. Anamnesis of the disease and clinical and paraclinical peculiarities of the course of the disease with consideration of liver affection severity have been studied in 50 children aged 5–17 years. Results. It was estimated that in 52% of children the disease had a form of chronic hepatitis with minimal clinical symptoms, 44% of patients had liver cirrhosis with predominant signs of edematous and ascitic syndrome and 4% had fulminant hepatitis in a debut of the disease. Complicated family anamnesis was detected in 4% of patients. Syndrome of cytolysis predominated in patients with liver damage in the form of chronic hepatitis (р<0.05). Typical biochemical changes in cirrhotic patients were hepatocellular insufficiency with hypoalbuminemia and coagulopathy, hyperbilirubinemia and minimal hypertransaminasemia (р<0.05). Children with cirrhosis in contrast to patients with chronic hepatitis had reliably lower serum ceruloplasmin concentration and reliably higher excretion of copper with urine. Specific for Wilson disease Kayser—Fleischer rings were found out in 36% of patients only. Conclusions. Wilson's disease in children is characterized by progressing course in the form of either chronic hepatitis or liver cirrhosis or, rarely, fulminant hepatitis. Clinical and paraclinical symptoms of the disease are estimated by the severity of liver affection and vary from a course with minimal symptoms in patients with chronic hepatitis to edematous and ascitic syndrome and hepatocellular insufficiency in patients with cirrhosis and fulminant hepatitis. The research was carried out in accordance with the principles of the Helsinki declaration. The study protocol was approved by the Local Ethics Committee of all participating institution. Purpose — to investigate the vitamin D supply in children with chronic viral hepatitis B (HBV) depending on the activity of hepatitis and the stage of liver fibrosis. Materials and methods. Fifty children with HBV were examined. All children underwent a comprehensive examination according to the recommendation and elastography of the shear wave of the liver parenchyma to determine the stage of fibrosis. Serum 25(OH)D concentration was determined to verify the diagnosis of vitamin D deficiency. The peculiarities of vitamin D supply in children with HBV, depending on gender, age, hepatitis activity and stage of liver fibrosis, were evaluated. The study included children who did not receive calcium and vitamin D for 6 months. Results. Children of senior scholl age predomnanted among the surveyed. HBV was more often registered in boys 66.0% (n=33), while in girls — only 34.0% (n=17) (χ2=10.24; p=0.01). Among the examined children significantly more often 66.0% (n=33) we observed HBeAg-positive chronic hepatitis, 18.0% (n=9) children were diagnosed with HBeAg-positive chronic infection, HBeAg-negative chronic infection were determined in 14.0% (n=7) of children. In the vast majority (70.0%) of patients with HBV, the concentration of vitamin D in the serum was reduced (χ2=16.0; p=0.01). The average concentration of 25(OH)D was 59.85 [13.4–181] nmol/l and was in the zone of insufficiency. The optimal concentration of 25(OH)D was found in 30.0% (n=15) of children, insufficiency in 42.0% (n=21), vitamin D deficiency was diagnosed in 14 children (28.0%). Gender, age and biochemical activity of hepatitis do not affect the level of vitamin D in the examined children with HBV (p>0.05). The analysis of vitamin D supply depending on the stage of fibrosis did not reveal differences in the median concentration of 25(OH)D between groups of children, but in all groups the number of patients with low concentrations of vitamin D prevailed. Conclusions. HBV in children is characterized mainly by a chronic course, with a predominance of the parenteral route of infection. In most patients with HBV, the concentration of 25(OH)D in the serum is reduced. There was no statistically significant difference between the levels of vitamin D in children with HBV depending on gender, age, and biochemical activity of hepatitis. All children with cirrhosis of the liver were deficient in vitamin D. The research was carried out in accordance with the principles of the Helsinki declaration. Запальні захворювання кишечника (ЗЗК)-це група хронічних захворювань, які характеризуються деструктивним неспецифічним імунним запа ленням стінки кишечника, до основних форм належать хвороба Крона та неспецифічний виразковий коліт. Особливу увагу слід приділяти поза кишковим проявам, які можуть виявлятися у пацієнтів із ЗЗК одночасно з ознаками ураження кишечника, а в деяких випадках навіть домінува ти у клінічній картині. У статті оцінено частоту уражень суглобів у дітей з запальними захворюваннями в порівнянні з частотою імуноопосередкованих уражень шлунково кишкового тракту у хворих на ювенільний ідіопатичний артрит (ЮІА). Серед спостережених дітей з діагнозом «Ювенільний ідіопатичний артрит» у 4,27% спостерігалися клінічні ознаки ураження шлунково кишкового тракту. Оскільки частина субваріантів ЮІА патогенетично наближені до ЗЗК, можна припустити, що невідповідність поширеності ЗЗК у дітей обумовлена маскуванням їх позакишковими проявами. У випадках, коли прояви суглобового синдрому передують ураженням кишечника, такі пацієнти тривалий час спостерігаються як хворі на ентезит артрити.У близько третини дітей із встановленим діагнозом ЗЗК спостерігався суглобовий синдром, переважно у вигляді артралгій, лише у 7,5% мали місце клінічні прояви (наб ряк, болючість, порушення функції, болючість суглобів) та інструментальні прояви артриту. У 2 пацієнтів суглобовий синдром передував виникненню скарг з боку ШКТ. Недооцінка тяжкості позакишкових проявів штучно занижує ступінь активності запального процесу при ЗЗК, що може призводи ти до важких наслідків. Проаналізовано особливості перебігу, діагностики та терапії захворювання з важким ураженням шлунково кишкового тракту та суглобів у дитини з хворобою Крона. Пацієнтам із ЗЗК і суглобовим синдромом необхідно якомога раніше призначати лікування генно інженерними препаратами біоло гічної дії, що може надати змогу попередити важкі ускладнення та зберегти якість подальшого життя хворих дітей. Болезнь Вильсона - это заболевание, течение и клиническая картина которого варьируется от бессимптомных форм до тяжелых проявлений дисфункции многих органов и систем, диагностика которого требует мультидисциплинарного подхода. В большинстве случаев заболевание проявляется поражением печени, которое может возникать у детей старше 2 лет с такими патологическими состояниями, как острый гепатит, гепатомегалия, стеатоз печени, острая печеночная недостаточность с гемолизом, портальная гипертензия и даже декомпенсированный цирроз печени с асцитом. Ранняя диагностика и своевременно назначенная терапия предотвращают прогрессирование заболевания и инвалидизацию пациента. Основными методами диагностики болезни Вильсона являются биохимическое определение метаболизма меди (сывороточный церулоплазмин и суточная экскреция меди с мочой), молекулярногенетическое исследование и пункционная биопсия печени с исследованием концентрации меди в паренхиме печени. В настоящее время болезнь Вильсона лечат препаратами, которые способны блокировать всасывание меди в кишечнике (соли цинка), и препаратами, связывающими и выводящими излишки меди из организма - D-пеницилламином, триентином. Wilson’s disease is the disease, the course and clinical picture of which varies from asymptomatic forms to severe manifestations of dysfunction of many organs and systems and the diagnosis requires a multidisciplinary approach. The disease usually manifests itself with liver damage, which can occur in children older than 2 years with such pathological conditions as acute hepatitis, hepatomegaly, hepatic steatosis, acute hepatic failure with hemolysis, portal hypertension, and even decompensated liver cirrhosis with ascites. Early diagnostics and timely treatment prevent disease progression and disability of the patient. The main methods of diagnosing Wilson’s disease are the biochemical determination of copper metabolism (serum ceruloplasmin and daily excretion of copper in the urine), molecular genetic research, and puncture biopsy of the liver with the study of copper concentration in liver parenchyma. Nowadays, Wilson’s disease is treated with drugs that are able to block the absorption of copper in the intestine (zinc salts) and drugs that bind and remove excess copper from the body - D-penicillamine, Trientin.
