Ярославль Потеря конечности или ее сегментов -большая утрата и может привести не только к потере трудоспо-собности, но и нарушить жизнь человека в целом. Именно поэтому восстановление потерянной части тела интересовало человечество с давних времен. Начиная с экспериментов макрореплантации и трансплантации конечностей, медицина постепенно дошла до уровня аллотрансплантации конечностей в клинической прак-тике, а возможности современной микрохирургии позволили реплантировать отчлененные кончики пальцев или устранить дефекты дистальных фаланг пальцев кисти путем пересадки со стопы. В статье приведен крат-кий исторический очерк развития вопроса реплантации и трансплантации конечностей на примере клиниче-ского и экспериментального опыта ярославской школы.Ключевые слова: отчленение, реплантация, трансплантация, аллотрансплантация, конечность.Amputation of the limb or its segments is a big loss for a human and can lead not only to loss of ability to work, but also disrupt a person's life. That is why the restoration of lost body parts interested mankind since historic times. Since experiments on macroreplantation and limb transplants, medicine gradually reached the level of limb allograft in clinical practice, and the possibilities of modern microsurgery allow the replantation of amputated fingertips or elimination of the distal phalangeal defects by transplanting from the foot. The article gives a brief historical sketch of the development of limb replantation and transplantation on the example of clinical and experimental experience of the Yaroslavl School.

From 2005 to 2009 we treated 112 patients with fractures of distal segment of the humerus, 83 (74,10%) of them were injured when falling from the height of its own growth, 8 (7,14%) - in an accident, 6 (5,35%) - a fall from a height, 13 (11,6%) - in sports, and two - as a result of violent acts. In 97 (87%) cases the fractures were closed, in 15 (13%) - open. 19 patients were treated conservatively and 93 - operatively. Functional results were evaluated using the Mayo elbow performance score (MEPS) in 78,37% of patients. Results of the conservative method: excellent and good - 46,66%, and in all patients with type A without displacement - in 100%. The fair results - in 33% and the poor in 20% - with type В and C. The results of operative treatment: excellent and good - 90,41%, the fair results - in 9,58%, no poor results. Initial prevent the oedema and bleeding into joints and soft tissues, adequate rigid fixation of fractures with the maximal restoration of the elbow joint congruent without additional immobilization and early active mobilization is essential to prevent the elbow stiffness and achieve a good outcomes.
The experience of treatment of patients with intra-articular fractures of proximal segment of the tibia is presented. For improvement of diagnostics the body section radiography was used. Conservative methods were applied in 146 patients, surgical - in 202. The best results were achieved on application of dampening skeletal traction - 88,2%.

Революционная, д. 5) По территории Архангельской области пролегает двухполосная федеральная автомобильная дорога (ФАД) М8 «Холмогоры» с присущими ей особенностями, характерными для северных трасс, обуслов ливающими возникновение дорожнотранспортных происшествий (ДТП) с медицинскими последствия ми и осложняющими своевременность и качество оказания медицинской помощи пострадавшим в них. Цель исследования -изучить медицинские последствия ДТП на рассматриваемых территориях арктиче ской зоны России и дать им медикосоциальную и медикотактическую оценку. Объектом исследования стали 327 историй болезни, оформленных на пострадавших в ДТП, поступивших в 2012-2014 годы по срочным показаниям в многопрофильные больницы г. Северодвинска (Архангельская область). Отмече но преобладание ДТП с медицинскими последствиями в летний период, превалированием в выходные дни и вечернее время (18.00-23.59 ч). Более 50 % от общего числа пострадавших составили лица моло дого и трудоспособного возраста до 40 лет. Среди пострадавших в ДТП на ФАД М8 преобладали води тели и пассажиры автотранспортных средств, а на дорогах г. Северодвинска -пешеходы. Выявлено, что на догоспитальном этапе персонал бригад скорой медицинской помощи практически не использовал у пострадавших инфузионную терапию. 235 (71,9 %) госпитализированных пострадавших лечили консервативно, а 92 (28,1 %) -требовали оперативных вмешательств. Приоритетными методами последних являлись металлоостеосинтез пластинами с угловой стабильностью и блокируемый интраме дуллярный остеосинтез. Проведенный анализ выявил необходимость повышения эффективности управ ления качеством оказания скорой медицинской помощи и лечения пострадавших в ДТП.Ключевые слова: чрезвычайная ситуация, дорожнотранспортное происшествие, пострадавший, травма, скорая помощь, Крайний Север, арктическая зона, Архангельская область.
