; 2 Новосибирский государственный технический университет, Новосибирск При позвоночно-спинномозговой травме (ПСМТ) полиорганная недостаточность констатируется в первые часы после повреждения с развитием жизнеугрожающих состояний на фоне нейрогенного шока. Важнейшей составляющей интенсивной терапии является лечение клинической формы кишечной недостаточности-пареза кишечника. Цель исследования: оценка способа прогнозирования/диагностики пареза кишечника на основе компьютеризированного анализа кишечных шумов по данным фоноэнтерографии и показателям внутрибрюшного давления (ВБД). На этапах интенсивной терапии пациентов с ПСМТ проводилась оценка степеней пареза кишечника по клиническим проявлениям (1-я группа, n=14; 2-я группа, n=14). Методы компьютерной фоноэнтерографии и измерения ВБД использовались только во 2-й группе. Также во 2-й группе исследования определяли частоту кишечных шумов (ЧКШ) и показатели уровней ВБД в режиме реального времени, вычисляли коэффициент паретичности (по формуле: Кр = ВБД/ЧКШ). В 1-й группе исследования степень тяжести пареза определяли только по клиническим данным. Мощность кишечных шумов во 2-й группе характеризовалась быстрым их затуханием при возникновении пареза ЖКТ с минимальными клиническими проявлениями кишечной недостаточности (КН) на 7-10-е сутки (р=0,003). Выявлена обратная корреляционная зависимость между ЧКШ и ВБД при r=0,8. Также установлена корреляционная зависимость между степенью пареза ЖКТ и Кр при r=0,78 (р=0,008). При увеличении текущего показателя Кр в сравнении с начальным более чем на 500% прогнозировали вероятность развития пареза ЖКТ и пересматривали тактику антипаретичной терапии. В результате исследования выявлена корреляционная зависимость между степенями пареза ЖКТ и ВБД при r=0,9 на 7-10-е сутки. Максимальные клинические проявления пареза были на 7-10-е сутки в обеих группах. Но степень тяжести пареза во 2-й группе была меньше, чем в 1-й, в результате своевременного изменения тактики интенсивной терапии после оценки субклинических проявлений пареза, что и лежит в основе предложенного способа прогнозирования и ранней диагностики пареза кишечника. Objective. To analyze the peculiarities of changes in intestinal microbiota in spinal cord injury.Material and Methods. The literature search was carried out for the period of 2012–2022 in the PubMed, ScienceDirect, eLibrary and GoogleScholar databases for the following keywords: spinal cord injury, microbiota, and dysbacteriosis. Out of 220 literature sources, 40 full-text articles were selected which analyze the influence of spinal cord injury on the state of the intestinal microbiota using an innovative method of metagenomic high-throughput 16S sequencing.Results. Literature data on the pathogenesis of organ dysfunction in spinal cord injury, as well as its impact on the state of the intestinal microbiota are presented. The results of experimental and clinical studies of the intestinal microbiota in spinal cord injury and its influence on the development and course of the pathological condition are reported.Conclusion. Understanding how the intestinal microbiota changes after spinal cord injury and what role it plays in potentiating inflammation or protecting the spinal cord from secondary injury and infections is very important in determining the strategy and tactics for managing patients. Possession of methods for correcting intestinal microbiota disorders in SCI is important in the treatment of such patients.
; 2 Новосибирский государственный технический университет, Новосибирск Проведен анализ данных, полученных в серии последовательных пациентов геронтологического возраста (n=250) при проспективном одноцентровом исследовании на этапе планирования и подготовки для первичного и ревизионного эндопротезирования на фоне сахарного диабета (СД) 2-го типа. Цель исследования: экспонировать зависимость между вариантами нарушений моторно-эвакуаторной функции желудочно-кишечного тракта (МЭФ ЖКТ) и показателями уровня гликемии у геронтологических пациентов с СД 2-го типа перед проведением плановой операции эндопротезирования тазобедренного или коленного суставов. У всех пациентов выявлена дисфункция ЖКТ на фоне инволютивных изменений слизистой оболочки, токсического действия НПВС и системной гипергликемии. Дисфункция регистрировалась в виде хронической кишечной недостаточности с изменением МЭФ ЖКТ. При регистрации кишечных шумов методом компьютерной фоноэнтерографии выделено 3 типа кишечной перистальтики. Пациенты (n=15) с сильным и умеренным типом перистальтики и компенсированными показателями гликемии оперированы в первые сутки с момента поступления. Пациенты со слабым типом перистальтики демонстрируют очевидную отрицательную зависимость между перистальтической активностью ЖКТ и уровнем гликемии (р<0,05; r=0,58). Зависимость между МЭФ ЖКТ и уровнем гликемии отражена в соотношении с условным названием «интестинальный пик-фактор» (ИПФ). Показатели МЭФ ЖКТ и уровень гликемии коррелируют с клиническими гастроэнтерологическими синдромами (p<0,002; r=0,7). Пациенты со слабой перистальтикой оперированы через 45±6 часов после предварительной предоперационной подготовки (энтеральное питание методом сипинга с применением гастрокинетиков) на фоне улучшения МЭФ ЖКТ и стабильных показателей гликемии. Показатель в виде ИПФ позволяет отслеживать состояние МЭФ ЖКТ и уровень гликемии в режиме реального времени. Это дает возможность контролировать дисфункцию ЖКТ как фактор риска при ортопедических операциях эндопротезирования суставов. Цивьяна, новосибирск, российская Федерация Цель: оценить результаты показателей гастроэнтерологического опросника GSRS в качестве экспресс-теста в выявлении дисфункции ЖКТ у больных старше 75 лет, длительно принимающих нестероидные противовоспалительные средства (НПВС) с кокс-и гонартрозами на фоне сахарного диабета (СД) II типа. Материал и методы: оценка выраженности гастроэнтерологических жалоб проведена с помощью опросника качества жизни GSRS у 100 пациентов старше 75 лет с кокс-и гонартрозами. I группа включала 60 пациентов с сопутствующим СД II типа, во II группу вошли 40 больных без СД. Результаты: констатация и анализ общеклинических данных в группах позволили рассчитать частоту клинических проявлений гастроэнтерологических синдромов и их средний числовой балл выявления по опроснику GSRS. Средний итоговый балл опроса был значимо выше в группе больных с кокс-и гонартрозами на фоне СД II типа. Констипационный, рефлюксный и диспептический синдромы оказались нетождественными в обеих группах и имели статистически значимые различия. Это подтвердило более выраженные нарушения моторно-эвакуаторной функции (МЭФ) ЖКТ в группе больных с кокс-и гонартрозами на фоне СД II типа. Заключение: с целью профилактики гипо-или гипергликемических состояний в периоперационном периоде у больных с кокс-и гонартрозами целесообразно использование гастроэнтерологического опросника качества жизни GSRS, который позволяет выявить угнетение МЭФ ЖКТ. Пациенты с кокс-и гонартрозами и сопутствующим СД II типа, длительно принимающие НПВС, имеют более выраженную дисфункцию ЖКТ. Основой критического состояния при позвоночно-спинномозговой травме шейного отдела позвоночника является повреждение спинного мозга, дисбаланс вегетативной нервной системы, с развитием нейрогенного и спинального шока, полиорганной недостаточности. Состояние гиперкатаболизма и гиперметаболизма при позвоночно-спинномозговой травме приводит к истощению всех запасов энергии, потере мышечной массы, снижению синтеза белка, потере целостности слизистой оболочки желудочнокишечного тракта, повышению восприимчивости к инфекциям, нарушению заживления ран и затруднению отлучения от ИВЛ. Адекватное восполнение белково-энергетической недостаточности и своевременная коррекция дисфункции желудочно-кишечного тракта является важным фактором восстановления пациентов после полученной травмы. Цель исследования -представить современные и исторические данные о нутритивной недостаточности и желудочно-кишечной дисфункции при позвоночно-спинномозговой травме. Поиск литературы осуществлялся в базах данных PubМed, Sciens direct Elibrary, Google Scholar и Scopus, а также использовалась информация, имеющаяся в прочих интернет-ресурсах с 1989 по 2021 г. по ключевым словам. Найдено 250 иностранных и 50 отечественных источников. Для анализа отобраны 44 статьи, из которых 70 % опубликованы за последние 10 лет. Проблема нутритивной недостаточности рассматривалась на фоне имеющейся дисфункции желудочнокишечного тракта при позвоночно-спинномозговой травме. Представлены результаты различных исследований. Проведен анализ данных по вопросам эпидемиологии и патофизиологии позвоночноспинномозговой травмы, патогенеза развития кишечной и нутритивной недостаточности у данной категории пациентов. Рассмотрены вопросы диагностики и современных методов терапии. Необходимо продолжить изучение состояния желудочно-кишечного тракта и нутриционного статуса при позвоночноспинномозговой травме для своевременной диагностики, профилактики и коррекции возникающего критического состояния.
