Este artículo está orientado a realizar una reflexión crítica con perspectiva de género acerca de la violencia intrafamiliar contra las mujeres en Bogotá. Se presentan datos que permiten dimensionar la problemática y se realiza una revisión de estudios que lleva al análisis de aspectos asociados al fenómeno. Los bajos niveles educativos y las precarias condiciones sociales de las mujeres al igual que la pareja como principal agresor resultan relevantes en este contexto donde el modelo patriarcal tiene aún una gran prevalencia.
Objetivo general: describir y analizar, desde una perspectiva cualitativa, las vivencias de un grupo de mujeres gestantes con diagnóstico de embarazo de alto riesgo con el fin de contar con criterios para construir intervenciones de enfermería que puedan responder a sus necesidades de cuidado. Materiales y métodos: se realizó una entrevista a profundidad, partiendo del marco teórico, a 10 mujeres de 17 a 39 años con embarazo de alto riesgo, atendidas por la Red Distrital de Salud. El análisis fue apoyado por el Atlas ti 6, basado en la Teoría Fundamentada. Resultados: las situaciones asociadas al alto riesgo en el embarazo encontradas en este grupo son: intervalo intergenésico —riesgo más frecuente—, hipertensión inducida por el embarazo, anemia, complicaciones obstétricas del embarazo anterior, antecedente de aborto a repetición, obesidad, bajo peso, placenta previa, tuberculosis, isoinmunización Rh, infección urinaria a repetición y consumo de sustancias psicoactivas (SPA). Se destacan los embarazos no planeados; la mitad de las participantes no cuentan con apoyo económico de su pareja; 7 perdieron su trabajo; 5 viven con parientes. La violencia de género y la pobreza limitan la toma de decisiones y afectan su salud. Se visualizan estrés y sentimientos de ambivalencia: por un lado, entusiasmo con su embarazo y, por otro, cambios que producen desequilibrio y afectan la calidad de vida. Conclusiones: las mujeres de este grupo social requieren de atención integral y oportuna, evitando demoras, y educación sexual y reproductiva. This paper conducted a critical review from a gender perspective, identifying selected studies addressing intimate partner violence against women during pregnancy. Seventy-two studies from 23 countries between 1994 and 2013 were analyzed. Intimate partner violence in pregnancy is associated with obstetric and birth complications, mental health consequences, and effects on the newborn as low birth growth. Countries with lower human development scores show higher rates of these ramifications. Socio-demographic conditions such as educational level, social status, age and marital status appear associated. While in most countries pregnancy reduces partner violence, there are exceptions suggesting that it is not necessarily a protective factor. The significance given to motherhood and pregnancy in different social groups and the level of presence of patriarchal relations may offer an explanation for this phenomenon. Psychological violence is the most frequently reported. Research must strengthen the qualitative approach. Ongoing issues in contexts where intimate partner violence increase and where physical violence prevails during pregnancy require to be studied. It is imperative to train health providers to detect and prevent intimate partner violence. Se realizó un estudio de tipo cualitativo basado en 6 entrevistas y otras estrategias narrativas, aplicadas a mujeres con cáncer de seno, entre los 42 y los 66 años. Estas mujeres eran provenientes de regiones rurales de Colombia y fueron acogidas por un albergue durante su estadía en Bogotá para su tratamiento en el Instituto Nacional de Cancerología. Mediante la construcción de historias de vida, el estudio revela que los diagnósticos en varios de estos casos fueron tardíos y que fueron las mismas mujeres quienes detectaron anomalías que las llevaron a consultar. Los aspectos relativos a la accesibilidad a la salud así como su precaria situación económica se destacan entre las principales limitaciones que estas mujeres han tenido que sortear. Buena parte de las participantes han sido intervenidas quirúrgicamente, mediante mastectomía, no siendo para ellas la reconstrucción del seno una prioridad. La caída del cabello afecta de manera signifi - cativa a este grupo de mujeres quienes requieren además minimizar otros efectos secundarios de la quimiote - rapia y la radioterapia como las afecciones del sistema digestivo y los problemas de la piel. Otras necesidades se relacionan con el tener una vida activa, recrearse y realizar actividad física. Sus familias y su espiritualidad representan sus mayores prioridades.
Objetivo: identificar la perspectiva de las gestantes atendidas en la Subred Integrada de Servicios de Salud Norte sobre la participación de la pareja masculina en el embarazo. Metodología: se realizó un estudio cualitativo basado en 5 grupos focales con 25 gestantes entre los 15 y 32 años, atendidas en 3 centros de la Subred Integrada de Servicios de Salud Norte; los resultados se analizaron mediante el software Atlas ti 8®. Resultados: se observa interés de las gestantes por la participación de la pareja masculina en el embarazo; dan importancia al apoyo afectivo, social y económico que esta pueda ofrecer a la embarazada y al establecimiento de un vínculo afectivo con el nonato. Conclusión: en una perspectiva de humanización, los servicios de salud sexual y reproductiva están llamados a superar los estereotipos de género que excluyen a los varones de los procesos reproductivos y a crear estrategias que consideren las necesidades de los padres y sus realidades; es posible fortalecer su vinculación convocándolos a sesiones en las que sus necesidades puedan ser reconocidas.
