Paciente de sexo femenino, 25 años, sin antecedentes mór-bidos, presenta lesión tipo placa verrucosa, congénita de coloración café, de aproximadanente 4 x 3 cm, ubicada en fosa ulnar derecha. Consulta por aparición de pequeños nódulos azul grisáceos de 3 años de evolución, que crecen bajo nevo verrucoso antes descrito (Figuras 1 y 2 ). Se realiza biopsia incisional de la lesión que revela papilomatosis, hiperqueratosis, hipergranulosis, hiperplasia lentiginosa melanocítica en la epidermis. Dermis con fascículos de melanocitos fusados pigmentados, sin atipias. Diagnóstico compatible con nevo epidérmico verrucoso asociado a nevo azul.Los nevos azules corresponden a una proliferación dérmica de melanocitos dendríticos intensamente pigmentados (nevo azul común), que en ocasiones se extienden hasta la hipodermis (nevo azul celular). A la dermatoscopía son lesiones con pigmentación azul homogénea, que varían del azul-grisáceo o marrón-azulado al negro-azulado.El nevo azul común se presenta típicamente como un nevo solitario que a menudo surge en la adolescencia, y con mayor frecuencia se encuentran en la superficie dorsal de las manos y los pies. El nevo azul celular tiende a ser un nódulo o placa más grande y más elevada, de al menos 1 cm de diámetro, con una superficie lisa o ligeramente irregular, que puede ser confundido con melanoma, dado las características de atipia que puede contener. Sin embargo, no hay consenso respecto a la clasificación o criterios diagnósticos para este tipo de lesiones y su espectro maligno.Los nevos verrucosos (nevos epidérmicos) son crecimientos benignos hamartomatosos de la piel que están presente desde el nacimiento o se desarrollan en la primera infancia. Ellos pueden estar compuestos de múltiples células y anexos epidérmicos.La distribución de los nevos epidérmicos es muy variable y tienden a seguir los patrones de migración de células precursoras en la piel conocidas como " líneas de Blaschko".Se presenta este caso clínico principalmente por la infrecuente asociación de nevos epidérmicos verrucosos y nevos azules, no existiendo en la literatura asociaciones descritas.
