The article presents the results of the treatment of inflammatory complications in patients with orthopedic constructions on dental implants using the course complex application of low-intensity infrared laser therapy and low-frequency electrostatic therapy. It has been shown that under the influence of a complex of physiotherapeutic factors, a more pronounced inverse dynamics of the clinical manifestations of peri-implantitis is observed in comparison with the standard treatment regimen, which has a positive effect on the indicators of the secondary stability of the implants themselves. The mechanisms of realization of the therapeutic efficiency of a low-intensity laser and a low-frequency electrostatic field are considered from the medical and biological point of view. The analysis of the study allows concluding that the course complex application of physiotherapeutic technologies in the treatment of post-prosthetic inflammatory complications during dental implantation is highly effective. The addition of the standard treatment regimen with the combined use of a low-frequency electrostatic field and laser therapy has a positive effect on the regression of the clinical manifestations of peri-implantitis and on the index indicators of the patients’ dental status. The increase in therapeutic efficiency observed when using a low-frequency electrostatic field in combination with an infrared laser of the complex is realized due to the potentiating type of interaction of physical factors, which is based on various points of application and the mechanisms of their corrective activity.
The article is devoted to the study of the dynamics of the quality of life of patients with peri-implantitis in the course of combined use of a low-frequency electrostatic field and low-intensity infrared laser therapy. It has been shown that in the initial state, the quality of life of patients with peri-implantitis is significantly reduced in comparison with the group of healthy individuals. The high sensitivity of the «Chewing food» domain in relation to disease progression was established, which allows considering this domain as the effectiveness of the treatment provided. The course combined use of an infrared laser and a low-frequency electrostatic field in the treatment of post-prosthetic inflammatory complications during dental implantation, along with high clinical efficacy, has a maximum corrective effect on the quality of life of patients. Additional evidence of the obtained data, objectifying the subjective opinion of patients in relation to their quality of life, is the results of correlation adaptometry. This technique, based on the parameters of the correlation matrix, makes it possible to distinguish a state of stable homeostatic equilibrium, in which changes in regressor factors (clinical-functional, biochemical, psychological, and other variables) are not reflected at the level of the resulting trait, the function of which was performed by the quality of life. The use of correlation adaptometry confirmed the high efficiency of combined physiotherapy in relation to the quality of life of patients with peri-implantitis, which indicates the formation of a high level of functional reserves and adaptive abilities of the body.
В статье представлены результаты использования предиктивного подхода в определении биомаркеров, позволяющих осуществлять прогноз эффективности терапии больных с постпротетическими воспалительными осложнениями при дентальной имплантации. Предиктивный подход, учитывающий генетические, эпигенетические, протеомные, метаболомные и метагеномные маркеры, а также вариативные фенотипические признаки организма человека, во многом определяет реализацию персонифицированного применения технологий. В основе построения прогноза была принята математическая модель множественной регрессии. В качестве зависимой переменной был предложен коэффициент эффективности терапии, показавший высокую информативность. Сравнительный анализ коэффициентов эффективности терапии при применении различных схем лечения периимплантитов позволил установить, что технология курсового комбинированного использования низкоинтенсивного лазера и низкочастотной электростатической терапии проявляет максимальную эффективность, что дополнительно подтверждается результатами проведенной корреляционной адаптометрии. Построение математической модели множественной регрессии позволило выделить клинико-функциональный кластер предикторов, включающий параметры нутритивного кровотока, индексный показатель кровоточивости десневой борозды и коэффициент антиоксидантной защиты. Его высокая прогностическая информативность в отношении эффективности комбинированного применения лазера и низкочастотного электростатического поля позволяет определить показания и противопоказания к использованию данной схемы лечения пациентов с постпротетическими воспалительными осложнениями при проведении дентальной имплантации The article presents the results of using a predictive approach in determining biomarkers that allow predicting the effectiveness of therapy in patients with postprosthetic inflammatory complications during dental implantation. The forecast was based on a mathematical multiple regression model. The coefficient of effectiveness of therapy was proposed as a dependent variable, which showed high information content. A comparative analysis of the coefficients of the effectiveness of therapy with the use of various treatment regimens for peri-implantitis made it possible to establish that the technology of course combined use of a low-intensity laser and low-frequency electrostatic therapy exhibits maximum efficiency, which is additionally confirmed by the results of the correlation adaptometry performed. The construction of a mathematical model of multiple regression made it possible to identify a clinical and functional cluster of predictors, including parameters of nutritional blood flow, an index of bleeding of the gingival sulcus, and an antioxidant protection coefficient. Its high prognostic informational value in relation to the effectiveness of the combined use of a laser and a low-frequency electrostatic field allows one to determine the indications and contraindications for the use of this treatment regimen for patients with post-prosthetic inflammatory complications during dental implantation
BACKGROUND: Previous studies have shown high clinical efficacy of the course combined use of a low-frequency electrostatic field and low-intensity infrared laser therapy in patients with peri-implantitis. It was shown that the use of these physiotherapeutic factors is accompanied by the development of anti-inflammatory and analgesic effects, increased tissue blood flow in the area of therapeutic exposure, as well as activation of trophic-regenerative processes that determine the degree of osseointegration of implants. However, along with resolving the issue of the current relief of clinical manifestations of peri-implantitis, an important point is the study of the long-term effect of the treatment method used. Evaluation of long-term results makes it possible to determine the duration of the preservation of the therapeutic effect, which makes it possible to determine the optimal parameters of the frequency and frequency of the therapeutic effects. AIMS: Evaluation of complex physiotherapeutic treatment of patients with post-prosthetic inflammatory complications during dental implantation in the long-term period (after 6 and 12 months). MATERIAL AND METHODS: The study was carried out with the participation of 132 patients aged 35 to 57 years with a clinically and radiologically confirmed diagnosis of class I and II peri-implantitis. All patients were divided into 4 groups by the method of simple fixed randomization. The first group (control group, 31 patients) received standard dental treatment, including professional oral hygiene, removal of dental plaque and granulation, and debridement of peri-implant pockets. In the second group (main I, 31 patients), in addition to the standard therapy, a course of laser therapy was carried out using the BTL-4000 physiotherapy apparatus. In the third group (main II, 30 patients), the standard treatment was supplemented with a course of low-frequency electrotherapy. The fourth group (main III, 31 patients), along with standard treatment, received low-frequency electrotherapy in combination with laser therapy. Long-term results were assessed 6 and 12 months after the end of therapy using clinical-functional, biochemical and psychological methods. RESULTS: The course combined use of a laser and a low-frequency electrostatic field as a supplement to the standard treatment protocol for patients with peri-implantitis ensures the maximum preservation of higher indicators characterizing the restoration of the functions of the dentition, aesthetic parameters of the face and an increase in the quality of life of patients in general. CONCLUSION: Taking into account the risk of developing a possible exacerbation of the inflammatory process, as well as the dynamics of the studied clinical and functional parameters in patients with peri-implantitis in the long-term period, it was concluded that it is necessary to conduct repeated courses of combined use of an infrared laser and low-frequency electrostatic therapy in 69 months.
