Functional appliances are widely used for regulating and directing growth in which commonly used are Frankel regulator and twinblock. Frankel regulator expands the orofacial arena for allowing growth by passive expansion, whereas twin block aids sagittal correction in retrusive mandible. Heres, an attempt to bring about both the changes in a single appliance where sagittal correction as well as keeping away constricted musculature is desired. In this appliance, passive and active expansion has been used simultaneously for better results. In the hindsight of the results, we could get in this case using Twinkle R 3D appliance, we hope that this appliance will be helpful in many more similar cases and act as adundum in clinician's armamentarium.

Background: Maxillary anterior teeth play a crucial role in aesthetics, phonetics, and mastication. For successful orthodontic treatment evaluating the morphology of the alveolar bone and incisive canal would help in avoiding root resorption, dehiscence, and fenestration. This study is aimed to research the configurational relationships among maxillary incisors, alveolar bone, and incisive canal through Cone Beam Computerated Tomography (CBCT). Methods: CBCT images of 35 orthodontic patients were evaluated for length of the canal (L); angles between the palatal plane and the maxillary alveolar border (01),the incisive canal (02), and maxillary incisor (03); distance from the right maxillary incisor to the incisive canal (D). All the measurements were performed on sagittal plane with the exception of (D) which was made on axial plane. Statistical analysis was performed on the above parameters using two sample test and Pearson’s correlation analysis. Results: There was no statistically significant difference between males and females for all the variables although there were large interindividual variation. There was a positive moderate correlation between 01 and 02 (0.480), 01 and 03 (0.487), 02 and 03 (0.345). The mean value for L and D were 10.38mm and 4.14mm respectively. Conclusion: There exists a large interindividual variability for incisive canal, proximity of incisors with that of incisive canal which could not be precisely predicted by the conventional cephalograms. The results of the study could be helpful clinically in planning orthodontic treatment for significant intrusion and retraction of maxillary incisors

Aims and Objectives: To assess the posed and dynamic smile and to compare the various attributes of smile in frontal, oblique and sagittal dimensions, in two different age groups (10- 15years and 18-25 years). Materials and Methods: The posed and dynamic smile parameters were measured using digital video clips in 80 subjects of two different age groups (10-15years and 18-25 years). Total of 15 parameters were studied in 3 planes of space. The data was analysed using student`s t-test to compare smile parameters across the age groups, paired t-test was used to analyse the parameters of posed and unposed smile within the same age group and chi-square test was performed for the discrete data. Results: The present study revealed significant differences in dynamic smile parameters between the two age groups. The parameters like Philtrum height and Smile index are more in older age group whereas the buccal corridor was more in younger age group. Significant differences were also recorded in various parameters in both the groups when the posted smile is compared with that of the dynamic smile. Conclusion: In both the age groups, the dynamic and posed smile attributes are significantly different, except for buccal corridor and interlabial gap.
