За последнее десятилетие стало очевидно, что не все случаи диабета, развившегося в детском, подростковом и более старшем возрасте, являются диабетом 1 и 2 типов. У пациентов удается чаще идентифицировать MODY и LADA тип диабета, а также более редкие синдромальные формы. В статье описан современный взгляд на этиологию, патогенез, клинику и диагностику MODY и LADA диабетов, что дает возможность точной постановки диагноза и назначения пациенту патогенетической терапии.
П атология щитовидной железы является одной из наиболее распространенных у человека. По данным ВОЗ, патологией щитовидной железы (ЩЖ) страдают более 200 млн человек. За последние 5 лет абсолютный прирост числа вновь диагностированных заболеваний в экономиче-ски развитых странах составил 51,8% среди женщин и 16,7% среди мужчин [1]. По данным Министерства здравоохра-нения Украины, за последние 5 лет количество больных с дисфункцией ЩЖ увеличилось в нашей стране в 5 раз. На диспансерном учете состоят 3,5 млн человек, 70% населения страдают дефицитом йода [2]. Чаще всего диагностируют зоб I-III степени -у 66,2% больных, узловые формы зоба -у 12,7%, тиреоидиты (аутоиммунный, острый, подострыйФактором риска сердечно-сосудистых заболеваний является дисфункция ЩЖ. Гипофункция ЩЖ отягощает механизмы потенцирования сердечно-сосудистых факторов риска: ожирения, артериальной гипертензии (АГ), нару-шений липидного и углеводного обменов. Сегодня компо-нентом метаболического синдрома (МС) или синдромa X (G. Reaven,1988 г.) являются атерогенная дислипидемия, повышенный уровень артериального давления (АД), инсу-линорезистентность, избыточная масса тела. Атерогенная дислипидемия при МС включает совокупность нарушений липидного обмена, таких как повышение уровня тригли-церидов и аполипопротеина b, рост количества мелких плотных частиц липопротеинов низкой плотности (ЛПНП) и снижение уровня холестерина (ХС), липопротеинов высокой плотности (ЛПВП) [3].Функциональное состояние ЩЖ оценивают по уровню тиреотропного гормона (ТТГ), а также периферических -свободных Т4 и Т3. Наиболее чувствительным показателем состояния ЩЖ является ТТГ. Американские эндокрино-логические организации рекомендуют использовать его как универсальный скрининговый показатель, который помогает обнаруживать манифестные и субклинические нарушения (анализ первого уровня, не дороже 5$) [4].Более изучено сегодня влияние на сердечно-сосудистые заболевания (ССЗ) манифестного гипотиреоза, чем субкли-нического гипотиреоза (СГ). По данным разных авторов, распространенность СГ составляет от 1,2 до 15% попу-ляции в зависимости от пола и возраста обследованных, манифестный гипотиреоз обнаруживают только в 2-3% [4]. По данным Американской ассоциации эндокринологов, СГ наблюдают у 3% мужчин и 10% женщин, а в возрастной группе старше 60 лет -20%. Значимость этой патологии обусловлена тем, что на протяжении одного года у 5-15% пациентов СГ переходит в манифестное состояние, причем в 70% случаев -с появлением симптоматики со стороны сердечно-сосудистой системы [5] По данным Всемирной организации здравоохранения, патология щитовидной железы -одна из наиболее распространенных у человека и является одним из факторов риска сердечно-сосудистых заболеваний. Гипофункция щитовидной железы отягощает меха-низмы потенцирования сердечно-сосудистых факторов риска, что послужило поводом для исследования уровня липидов крови у всех больных с дисфункцией щитовидной железы.
