Controlling infection is crucial in treating patients with acute pancreatitis (AP). The infectious process in AP often predisposes to subsequent sepsis by damaging not only the pancreas, but retroperitoneal tissues as well. Among other AP-associated factors, are the rapidly developing immune imbalance, the poor penetration of antimicrobial agents into necrotic tissue, and the impossibility of a single surgical debridement. Antibacterial and antifungal therapy for patients with infected necrosis and AP-associated extra-pancreatic infections remains a complex and largely unresolved problem, partially due to the high occurrence of multiresistant pathogens. The preventive use of antimicrobial agents has been discussed in the literature; however, the lack of consistent results makes it difficult to develop a unified strategy and clinical guidelines on this specific issue. Recent meta-analyses provide no conclusive evidence that antibacterial prophylaxis reduces the infection rate, mortality, or the need for surgical treatment in patients with necrotizing pancreatitis. We found only two studies indicating the benefits of using carbapenems for prophylactic purposes and one meta-analysis indicating a reduction in mortality under antibiotic treatment started no later than 72 h after the onset of the attack. Selective bowel decontamination is considered as one of the preventive anti-infection measures, although the available data may not be fully reliable. The main indications for antibacterial therapy in patients with AP are confirmed infected necrosis or extra-pancreatic infection, as well as clinical symptoms of suspected infection. Intra-arterial administration or local treatment with antibiotics can increase the efficacy of antibacterial therapy. No randomized studies on antifungal prophylaxis in AP are available; some reports though recommend using such therapy among patients at high risk of invasive candidiasis.
Traumatic pancreatic injuries are associated with significant morbidity and mortality. The review analyses recent studies on epidemiology, classification, diagnosis and treatment of patients with pancreatic injury. Pancreatic injury in abdominal trauma is observed from 3% to 12% of cases, the overall mortality ranges from 0% to 31% and is largely determined by damage to other organs. The most widely accepted grading system for defining categories of pancreatic injuries is Organ Injury Scaling developed by the American Association for the Surgery of Trauma. A high index of suspicion is necessary for early diagnosis, since in the first hours after the injury clinical manifestations and laboratory parameters are not specific, and changes in the CT picture do not correlate to the severity of the injury in 20-40% of cases. Contrast-enhanced CT is the first-line diagnostic tool in hemodynamically stable patients. MRI with MRCP and ERCP is used to evaluate the integrity of the pancreatic duct. Nonoperative management is recommended for hemodynamically stable grade I and grade II pancreatic injuries. Patients with pancreatic duct injury require predominantly surgical treatment. The preferred type of surgery for grade III and more severe injuries (distal resection, drainage of the damaged area combined with endoscopic drainage and stenting, or organ-preserving surgery in the form of pancreaticojejunostomy, pancreaticogastrostomy) remains controversial. In case of pancreatic injury proximal to the venous confluence, drainage of the damaged area is the only fairly safe option for the patient, indications for pancreaticoduodenal resection are limited to concomitant injury of the bile duct, duodenum with involvement of the major duodenal papilla, massive bleeding from crushed tissues of the pancreatic head. Larger prospective studies are warranted for better management of patients with pancreatic trauma.
The paper presents an analysis of the recent studies on the various aspects of surgical management of acute (excluding biliary) pancreatitis. It evaluates the suggestion of interventions in the sterile phase, which are limited to and include enzymatic peritonitis and abdominal compartment syndrome. Surgery is suggested when conservative treatment is ineffective, pain is present, which is associated with pancreatic fluid accumulation, there is a risk of the pancreatic fluid leaking into the abdominal cavity, or compression of the adjacent organs develops due to the disconnected pancre-atic duct syndrome. Infected necrosis is the main indication for surgical intervention in acute pancreatitis. The drainage is preferably delayed for at least 4 weeks following the onset of the disease, and is gradually performed (in a “step-up” manner). The choice of drainage technique is based on the necrosis localization, delimiting wall, surgeon’s expertise, and technical capabilities. Sequestrectomy can be performed starting from mini-invasive percutaneous drainage under endoscopic guidance, or using a covered metal stent. In the cases of early infection or advanced injury of retroperitoneal tissue, it is advisable to combine percutaneous and endoscopic methods, and use multiple transluminal gateway techniques with several draining tracts installed from single or multiple points of access.
Актуальность. Тактика ведения больных с жидкостными скоплениями при остром панкреатите четко не определена.Оценка результатов лечения на основании единых установленных классификационных критериев позволит более четкоопределить показания к различным видам оперативных вмешательств у данной категории больных.Цель исследования Анализ результатов лечения больных острым панкреатитом с различными типами жидкостных ско-плений согласно пересмотренной классификации Атланты.Материалы и методы. 109 пациентов с острым панкреатитом и формированием жидкостных скоплений, которые раз-делены на типы согласно пересмотренным критериям Атланты (2012). Интерстициальный панкреатит был у 7 человек,некротический – у 102. Острые жидкостные перипанкреатические скопления отмечены в 5 случаях, панкреатическиепсевдокисты – в 2. Острые некротические жидкостные скопления были у 93 лиц. С диагнозом отграниченного некрозабыло госпитализировано 9 больных. Показаниями к хирургическому лечению было нагноение жидкостных образований,в отсутствие панкреатической инфекции – стойкий болевой синдром, быстрый рост объема жидкостной структуры.Сравнение групп проведено с использованием двустороннего точного критерия Фишера, критерия χ2, уровень значимостиотличий принят ≤0,05.Результаты. При интерстициальном панкреатите в результате консервативных мероприятий в 2 случаях достигнутрегресс жидкостных структур, включая панкреатическую псевдокисту, в 2 – значительное уменьшение размеров. Принаружном дренировании регресс достигнут у 2 лиц с острыми перипанкреатическими скоплениями. В случае панкреатиче-ской псевдокисты возник рецидив после удаления дренажа. При консервативном лечении острых некротических скопленийу 20% больных они регрессировали, у 40% – сформировался отграниченный некроз, у 36% – отмечалось увеличение размеровили нагноение, у 4% – уменьшение размеров. Дренирование острых некротических скоплений под УЗИ-контролем выпол-нено у 16 больных, открытым доступом – у 17. Сравнение результатов лечения оперированных малоинвазивным и откры-тым путем не выявило значимых отличий летальности (р=0,224) и частоты регресса (р=0,728). В группе отграниченногонекроза (46 человек) у 12 лиц операции были выполнены открытым путем, у 10 – под УЗИ-контролем. Частота регрессабыла выше при открытых операциях по сравнению с пункционным дренированием (р=0,027). У 24 больных с отграничен-ным некрозом (52%) консервативное лечение привело к стабилизации или уменьшению размеров образования, стиханиюболей, исчезновению признаков воспаления. Среди них на фоне активного врачебного наблюдения, в сроки от 4 месяцев до1 года, отграниченный некроз регрессировал у 8 лиц.Выводы. Формирование жидкостных образований при остром панкреатите требует продолжения консервативных ме-роприятий. В большинстве случаев оперативное лечение показано при нагноении. Пункционное дренирование под УЗИ-контролем является операцией выбора при остром некротическом скоплении. При отграниченном некрозе большей эффек-тивностью обладает открытое внебрюшинное дренирование.Ключевые слова: острый панкреатит, жидкостные скопления, пересмотренная классификация Атланты, дренированиепод УЗИ-контролем, открытый внебрюшинный доступ.
Êëèíè÷åñêèå íàáëþäåíèÿ Цель. Проиллюстрировать сложность идентификации микробов-возбудителей катетер-ассоциированной инфекции желчевыводящих путей, показать различия в чувствительности к антибиотикам микроорганизмов, находящихся в биопленочной и планктонной формах. Материал и методы. Пациент с псевдокистой головки поджелудочной железы, осложненной механической желтухой, оперирован двухэтапно. Выполнена холецистостомия под контролем УЗИ, в дальнейшем-холецистэктомия, резекция головки поджелудочной железы с формированием цистопанкреато-и гепатикоеюноанастомоза на выключенной петле тощей кишки, дренирование общего желчного протока через культю пузырного протока. Течение послеоперационного периода осложнилось рецидивирующим холангитом. Проведен микробиологический анализ отделяемого по дренажу с определением чувствительности к антимикробным препаратам традиционным способом. Поверхность дренажа исследована с помощью сканирующей электронной микроскопии. Для идентификации возбудителей использована масс-спектрометрия. Осуществлено моделирование биопленок на основе изолятов, выделенных с катетера, определена чувствительность микроорганизмов в составе биопленки. Результаты. При исследовании отделяемого по дренажу выделены Pseudomonas aeruginosa и Candida аlbicans. На поверхности дренажа выявлена биопленка, образованная не только двумя перечисленными возбудителями, но и микроорганизмами семейства Streptococcaceae. Штамм P. aeruginosa был не способен образовывать биопленку, но мог "встраиваться" в биопленку, сформированную C. albicans. В составе биопленки синегнойные палочки и кандиды утратили чувствительность к антимикробным препаратам, имевшуюся при существовании их в планктонной форме. Заключение. Установка катетера в желчные протоки связана с формированием на его поверхности биопленки. Рутинное микробиологическое исследование не всегда позволяет оценить спектр микробов-возбудителей. Традиционно определяемая чувствительность к антибиотикам планктонных форм не соответствует таковой при нахождении микроорганизмов в биопленке. Наружное желчеотведение ликвидирует холестаз, но не бактериохолию. Адекватное лечение инфекции желчевыводящих путей возможно при учете всех перечисленных факторов, важнейшим условием является восстановление оттока желчи в тонкую кишку.
The main indication for surgical treatment in acute pancreatitis (AP) is infected necrosis in the late phase of the disease. However, some patients require surgery in the sterile stage including the earliest period. In sterile necrosis interventions are associated with the risk of infection and increased mortality, being the responsibility of a surgeon. Patients with biliary AP often require early interventions and specific tactic regarding the pathology of the biliary tract. The review analyses recent studies and modern practical recommendations on the surgical treatment of patients with AP in the sterile stage and in biliary etiology. Obstruction of bile duct and cholangitis, enzymatic peritonitis, abdominal compartment syndrome caused by ineffective conservative measures, massive pancreatogenic accumulations with the threat of a rupture into the abdominal cavity, pain syndrome, compression of the adjacent organs often resulted from the disconnected pancreatic duct syndrome require invasive interventions to prevent the development of infectious necrosis. A number of questions have an ambiguous solution. The threshold level of intra-abdominal pressure, the rate of its increase, or other apparent criteria for the implementation of surgical decompression and the choice of decompression technique for abdominal compartment syndrome have not been stated. The optimal timing and the priority method of drainage in the disconnected pancreatic duct syndrome remain uncertain. The management of patients with biliary acute pancreatitis regarding prophylactic endoscopic papillotomy is controversial. Further, well-designed research studies involving surgical treatment of patients with acute pancreatitis are needed.
