Results of comparative study with participation of 69 patients (42-80 years old) with degenerative dystrophic changes of the knee joint are presented. In control group (n=46) only arthroscopic debridement was performed. In the main group (n=23) arthroscopic intervention was supplemented with insertion of arthromedullar implant that ensured the penetration of intraosseous content (artromedullary shunting) into joint cavity. Using the model reaction of cumene oxidation the radical-forming ability of wear particles of some orthopaedic alloys and corundum ceramics were studied. It was revealed that wear particles from different alloys catalyzed cumene oxidation whereas particles of corundum ceramics were inert. Cobalt alloy wear particles were much more active as compared with the titanium alloy and stainless steel particles. Production of cobalt alloy by laser sintering reduced the radical-forming ability of its wear particles. Activity of cobalt wear particles was preserved for a long time after their formation and could considerably strengthen the development of oxidizing stress at joint arthroplasty. Detection of catalytic activity of orthopaedic alloys' wear particles requires studying of the influence of free radicals generated by tribochemical reaction upon implant components and biological tissues.
Results of in vitro study of argentum containing tricalcium phosphate (TCPh-Ag) are presented. It is shown that biomaterial does not possess radical forming activity. Argentum containing forms of TCPh render bacteriostatic effect upon Staphylococcus haemolyticus and Escherichia coli. Dose-dependent effect of TCPh-Ag in relation to antibacterial and cytotoxic properties is demonstrated. It is determined that TCPh with rated substitution 0.5 is characterized by moderate cytotoxicity with preservation of antibacterial properties.
