Early vitrectomy (fewer than 3 weeks postoperatively) was associated with better visual results, while late vitrectomy resulted in limited visual acuity in a high percentage of patients and increased the risk for glaucoma and retinal detachment.
Silicone tubing was used to repair the patency of the nasolacrimal drainage system in 115 patients (four bilaterally) with chronic nasolacrimal duct obstruction. The patients were over 20 years old and 16 were men and 99 women; 46% presented an uncomplicated obstruction with epiphora and 54% presented additional signs of chronic dacryocystitis. We used the technique of Quickert and Dryden with modifications. The silicone tube remained for a mean of 5.5 months (+/- 2.5 SD). After removal of the tube the follow-up ranged from 6 to 42 months (mean +/- SD: 16 +/- 9). Patency was maintained in 89% of cases with uncomplicated obstruction and in 69% of those with chronic dacryocystitis (total success rate 78%). We can therefore recommend silicone intubation of the lacrimal drainage system as an alternative to dacryocystorhinostomy (DCR), in adults with obstruction of the nasolacrimal duct. In addition, unlike DCR this technique is bloodless and the operation takes less time.
Στην παρούσα μελέτη, εγένετο διασωλήνωση ολοκλήρου του αποχετευτικού συστήματος με tube σιλικόνης σε 84 περιπτώσεις ενηλίκων με απόφραξη του ρινοδακρυϊκού πόρου. Η διασωλήνωση εγένετο υπό τοπική αναισθησία με την βοήθεια ωτορινολαρυγγολόγου και με μέθοδο που βασίστηκε στην τεχνική του Quickert, μετά από προσωπικές μας τροποποιήσεις. Μετά χρόνο παρακολούθησης από την αφαίρεση του tube 25±17 μήνες και μέσο όρο χρόνου παραμονής του tube σιλικόνης 4 1 /2±2 μήνες, σε 54 περιπτώσεις, ο ρινοδακρυϊκός πόρος παρέμεινε απόλυτα βατός (πλήρης αποκατάσταση) και σε 8 περιπτώσεις η διάβαση του υγρού γινόταν δύσκολα, υποκειμενικά όμως, οι ασθενείς ανέφεραν σαφή βελτίωση της δακρύρροιας (συνολική επιτυχία 73%). Εάν λάβουμε υπ' όψη τα περιστατικά απόφραξης του ρινοδακρυϊκού πόρου χωρίς πύασκο (36 περιπτώσεις), τότε το ποσοστό επιτυχίας ανέρχεται στο 83%. Το υψηλό ποσοστό επιτυχίας της χρησιμοποιηθείσης μεθόδου διασωλήνωσης του δακρυϊκού αποχετευτικού συστήματος με tube σιλικόνης, σε απόφραξη του ρινοδακρυϊκού πόρου στους ενήλικες, αποδεικνύει την χρησιμότητα της εύκολης και αναίμακτης αυτής μεθόδου διασωλήνωσης σαν εναλλακτική λύση της ασκορινοστομίας.
