To investigate the relationship between the pattern of growth factors, cytokines and microcirculation state with the clinical status of coronary heart disease patients after coronary artery bypass grafting against the background of low-intensity laser radiation included in the standard rehabilitation complex in the early postoperative period. The study involved 52 patients with coronary artery disease requiring surgical treatment. The patients were randomized into two groups: observation and comparison. All patients received a complex of physical rehabilitation, and in the observation group there was an additional course of laser therapy. Questionnaires, assessment of hemodynamic parameters, quality of life, cognitive functions, psychoemotional state, laboratory and instrumental methods of investigation were performed at the I and II stages of the study. The most significant protective changes in response to laser therapy were observed in the myogenic circuit of the microcirculation. A strong direct correlation between the state of the myogenic circuit of the microcirculation regulation and the concentrations of HGF, TNFa and IL-10 was found during laser therapy. A direct moderate correlation between the parameters of myogenic, neurogenic and endothelial circuit and VEGF reflects a protective mechanism of lowintensity laser radiation. The study found a significant increase in the concentration of TGF b1, which may also indicate the protective effect of low-intensity laser radiation. Thus, the inclusion of laser therapy in the complex rehabilitation of patients after coronary artery bypass grafting allows to improve microcirculation, which is associated with changes in serum concentration of growth factors and cytokines.
Целью исследования было провести клиническое исследование влияния сульфидной бальнеотерапии на клинические проявления, гемодинамику, микроциркуляцию и функцию эндотелия у пациентов с артериальной гипертензией и предиабетом. Обследовано 82 пациента с артериальной гипертензией (АГ) и предиабетом, которые методом рандомизации разделены на две группысравнения (42 пациента) и наблюдения (40 пациентов). Все пациенты, включенные в настоящее исследование, получали стандартизованную медикаментозную терапию согласно клиническим рекомендациям. Пациенты группы наблюдения дополнительно получали комбинированную сульфидную бальнеотерапию в течение 14 дней. Лечебный комплекс состоял из 8 общих ванн и 7 сеансов аппликаций иловой сульфидной грязи Суксунского озера. Исследовательская программа (исходно и на 14-18 день бальнеологического лечения) предполагала оценку жалоб пациентов, качества жизни (КЖ) по шкале SF 36; проведение суточного мониторирования артериального давления (АД), оценку гемодинамики (импедансометрическим способом) на аппаратном комплексе «Эскулап», состояния микроциркуляции по данным Микротеста (ФМ Диагностика, Пермь, Россия). Комплексное лечение с включением сульфидной бальнеотерапии у больных АГ и предиабетом привело к уменьшению балла по шкале клинических проявлений, связанных с АГ и увеличением интегрального балла КЖ. На фоне тестового лечения у больных АГ и предиабетом наблюдается уменьшение индекса времени гипертензии по систолическому АД за сутки, по систолическому и диастолическому АД в ночные часы, а также по среднему значению диастолического АД ночью, значимое увеличение показателей, характеризующих податливость аорты, отмечается улучшение состояния эндотелиального и нейрогенного контура регуляции кожного микротока. Таким образом, сульфидная бальнеотерапияэффективный метод лечения АГ и предиабета, который может быть использован в дополнение к медикаментозной терапии.
