Бурденко, Москва Цель работы заключалась в сопоставительном анализе содержания α-дефенсинов (HNP1-3), про-и противовоспалительных цитокинов в плазме крови и цереброспинальной жидкости (ЦСЖ) и их роли в развитии системного воспалительного ответа в посттравматическом периоде у пациентов с тяжелой черепно-мозговой травмой (ЧМТ). Материалы и методы. В исследование включено 105 пациентов с тяжелой ЧМТ. Забор ликвора и плазмы крови проводили в 1-е и 7-е сутки после травмы. Количественное содержание α-дефенсинов (HNP1-3) в плазме крови и цереброспинальной жидкости, цитокинов (IL-1β, TNF-α, IL-6, IL-8, IFN-γ, IL-10) в сыворотке крови и ЦСЖ определяли методом ИФА с набором для определения человеческих α-дефенсинов (ELISAkit, HyCultbiotechnology, Нидерланды) и цитокинов («Вектор-Бест», Россия). Результаты. Проведенные исследования показали, что тяжелая ЧМТ сопровождается повышенной продукцией α-дефенсинов, про-и противовоспалительных цитокинов. В первые сутки после ЧМТ в плазме крови и ЦСЖ наблюдается значительное увеличение концентрации α-дефенсинов. В динамике на 7-е сутки отмечалось снижение содержания α-дефенсинов в плазме крови и ЦСЖ, более выраженное в ликворе. Между уровнем α-дефенсинов и содержанием белка, цитозом в ликворе в 1-е сутки, а также между α-дефенсинами и противовоспалительным цитокином IL-10 в плазме крови и ликворе на 7-е сутки выявлена прямая корреляционная зависимость, отражающая динамику течения воспалительного процесса в ЦНС. Анализ корреляционной связи между тяжестью ЧМТ по шкале ком Глазго (ШКГ), включающей оценку уровня сознания, и шкале исходов ком Глазго (ШИКГ) с исследуемыми показателями выявил статистически значимую сильную прямую корреляционную связь между концентрацией α-дефенсинов и IL-10 в ликворе на 7-е сутки и исходами тяжелой ЧМТ по ШИКГ, что позволило выделить 3 группы пациентов по тяжести течения и исходам ЧМТ. АннотацияЦелью работы является исследование цитокинового профиля сыворотки крови и цереброспинальной жидкости у больных с ЧМТ тяжелой степени. Материалы и методы. Для исследования были отобраны 20 пациентов с черепно-мозговой травмой по шкале глубины комы Глазго ≤8. Забор цереброспинальной жидкости (ЦСЖ) и сыворотки крови проводили в 1-е и 7 сутки после травмы. Содержание цитокинов (интерлейкины 1β, 6, 8, 10, ФНО-α, интерферон-γ) в сыворотке крови и ЦСЖ определяли методом твёрдофазного ИФА с набором реагентов ЗАО «Вектор-Бест» (Новосибирск). Результаты. Тяжелая черепно-мозговая травма сопровождается гиперсекрецией про-и противовоспалительных цитокинов, продукция которых в динамике развития воспалительного процесса, возникающего при травматическом поражении тканей мозга характеризуется значительным увеличением их концентрации в первые сутки и снижением в динамике на 7-е сутки после ЧМТ кроме ИЛ-6 и ИЛ-8, содержание которых увеличивалось в обеих биологических жидкостях в 1,5-2 раза соответственно. При этом уровень цитокинов в ЦСЖ достоверно превышал их сывороточные значения в среднем в 1,5 раза. Выявлена прямая корреляционная зависимость между уровнем ИЛ-6, ИЛ-8, цитозом и содержанием белка в ЦСЖ, свидетельствующая об их важной роли в патогенезе ЧМТ, что обосновывает целесообразность их исследования для прогнозирования течения и оценки эффективности лечения.
