Diabetes mellitus (DM) comprende un grupo de enfermedades metabólicas que tienen como factor común la presencia de hiperglucemia. España presenta estimaciones de prevalencia de DM tipo 2 que varían entre el 4,8 y el 18,7%, y de DM tipo 1 entre el 0,08 y el 0,2%, siendo un problema de salud importante debido a la comorbilidad asociada y los altos costes. La presentación del caso tiene como objetivo concienciar a los profesionales de la salud de la importancia de un diagnóstico precoz y adecuado, para optimización del tratamiento, además de plantear un diagnóstico diferencial de importancia para la práctica clínica habitual.
El aumento de la esperanza de vida de los pacientes diagnosticados de VIH gracias a los avances en el tratamiento antirretroviral en los últimos años ha puesto de manifiesto la importancia de un buen control de las comorbilidades crónicas y los factores de riesgo cardiovascular en estos pacientes. Tras su aparición, los primeros inhibidores de la proteasa fueron relacionados con la aparición de efectos adversos metabólicos, entre los que destacaba la hiperglucemia. Describimos un caso de un paciente que presenta un debut diabético en relación con el inicio del tratamiento antirretroviral con estos fármacos.
