The purpose of this research is to determine the intensity of biofilm formation, adhesion and motility of bacteria depending on different types of children's urinary tract infections. Materials and methods: the paper contains the research of adhesion, motility and biofilm formation properties of 44 clinical isolates of bacteria from children with lower urinary tract infections, acute and chronic pyelonephritis. The ability of bacteria to form biofilms was tested using the method of microtiter plates. The motility of the microorganisms was tested using standard methods. The ability of bacteria to adhere was investigated according to Brilis, through the use of formalinized erythrocytes. Results: differences were found in the ability to form biofilm between strains of bacteria that cause infections of the lower urinary tract (optical density 0.40 ± 0.06) and chronic pyelonephritis (optical density 0.48 ±0.07) (p<0.05). Furthermore, a correlation was established between adhesion and biofilm formation abilities of bacteria (r = 0.529 (p<0.01)). It was detected that increased motility ability of the bacteria reduces the force applied by this strain to form biofilm. Conclusions: bacteria isolated from children with acute and chronic pyelonephritis showed higher ability to adhere and form biofilm than the bacteria isolated from children with lower urinary tract infection. High ability to form biofilm and adhere of the isolated bacteria can be considered an adverse predictive factor in the course of urinary tract infection.
Актуальність. Інфекція сечових шляхів (ІСШ) — одна з основних причин госпіталізації дітей до стаціонару. Однією з основних причин неефективного лікування ІСШ у дітей є антибіотикорезистентність збудників ІСШ. Метою дослідження було визначення стану поширеності антибіотикорезистентності серед збудників інфекції сечових шляхів. Матеріали та методи. Дослідження антибіотикорезистентності проводилося на 61 клінічному ізоляті, що були виділені зі зразків сечі дітей, які були госпіталізовані з приводу захворювання на різні форми ІСШ. Визначення чутливості до 23 антимікробних препаратів здійснювалося за допомогою методу антимікробних дисків з подальшим визначенням зон затримок росту. Результати. Найбільшу ефективність щодо збудників інфекцій сечових шляхів мали представники групи аміноглікозидів (амікацин — до 91,9 % клінічних ізолятів, гентаміцин — до 82 %). Цефалоспорини ІІІ покоління мали різний рівень антибіотикорезистентності, що коливалася у межах від 11,8 % (цефтизоксим) до 42,5 % (цефподоксим). Зберігається високий рівень чутливості до синтетичних протимікробних препаратів: чутливість до фурагіну становила 89,8 %, до фосфоміцину — 86,5 %. Висновки. Рівень антибіотикорезистентності щодо збудників ІСШ значно варіює у різних антибактеріальних препаратів, а також залежно від форми ІСШ, тому проведення дослідження антибіотикорезистентності є важливим етапом в лікуванні хворих на ІСШ.
Urinary tract infection is one of the most common infectious diseases that affects people of any age and sex. The purpose of our study was to determine morphological changes in acute pyelonephritis, depending on the ability of bacteria strains to form biofilms. Materials and methods. The model of acute pyelonephritis was reproduced by ascending urinary infection model of laboratory mice with clinical isolates of E.coli with low biofilm formation ability (group # 1) and high biofilm formation ability (group # 2), the material was taken on day 7 of the experiment, histological sections were analyzed in comparison with control (group # 3). Results. In group # 1, leukocyte infiltration of the medulla of the kidney with the presence of edema of the stroma was observed. In group # 2, hyperinfiltration of all parts of the kidney was observed, together with the presence of extravasates and severe edema of the stroma. Discussion. Changes revealed by histological examination may indicate that biofilm formation leads to an ineffective immuneresponse, which in turn leads to increased secondary alteration. Conclusions. Biofilm formation is an important factor in the pathogenicity of microorganisms, which affects the course of acute pyelonephritis.
