В последние годы заболеваемость первичным раком печени увеличивалась. Самым распространенным гистологическим типом первичного рака печени является гепатоцеллюлярный. Ведущими факторами риска являются HBV и HCV, алкогольный цирроз печени, жировая болезнь печени. В России в 2018 г. выявлено 3695 случаев злокачественных новообразований печени и внутрипечёночных желчных протоков у женщин и 5115 у мужчин, что составляет 4,69 на 100 тыс. населения и 7,51 на 100 тыс. населения соответственно. Проанализированы непосредственные и отдаленные результаты хирургического лечения гепатоцеллюлярного рака. В исследовании принимали участие 35 пациентов. Из них мужчин -21, женщин -14, минимальный возраст -18 лет, максимальный -71 год. Средний возраст составил 49,5 лет. Послеоперационные осложнения составили 40 %. Из них чаще всего встречались абсцесс (14,29 %), желчный свищ (14,29 %), печеночная недостаточность (14,29 %), перфорация язвы двенадцатиперстной кишки (14,29 %). Послеоперационная летальность составила 8,57 %. Причинами летальных исходов были печеночная недостаточность, полиорганная недостаточность, перфорация язвы двенадцатиперстной кишки. 1-летняя выживаемость составила 97 %, 3-летняя выживаемость 59 %, 5-летняя выживаемость 47 %. Исследование показало, что оперативные вмешательства по поводу гепатоцеллюлярного рака при отработанной технике оперирования сопровождаются приемлемыми цифрами летальности и приемлемыми цифрами послеоперационных осложнений.Ключевые слова: первичный рак печени, гепатоцеллюлярный рак, послеоперационные осложнения, отдаленная выживаемость, 5-летняя выживаемость
