Zusammenfassung. Ein 21-jähriger Patient stellte sich im septischen Schock mit Multiorganversagen zwei Wochen nach einer Pharyngitis vor. Eine Thrombose der Vena jugularis interna, der Verdacht auf septische Embolien der Lunge und die positiven Blutkulturen bestätigten ein Lemierre-Syndrom – eine seltene Komplikation einer durch Fusobacterium necrophorum verursachten Pharyngitis mit hoher Morbidität und Mortalität, die sich als septische Thrombophlebitis der Vena jugularis interna mit hohem Embolierisiko präsentiert. Bei einer «Streptokokken-negativen» Tonsillitis mit Red Flags ist daher ebenfalls eine antibiotische Therapie indiziert.
