Alteración del estado de conciencia en el paciente intoxicadoAlterations in the state of consciousness in intoxicated patients / Alteração do estado de consciência no paciente intoxicado Marie Claire Berrouet Mejía 1 Natalia Eugenia Pino Ramos 2 Marcela Castro Botero 3 Ubier Eduardo Gómez Calzada 4 RESUMEN La disminución del estado de conciencia, específicamente el coma, es un motivo de consulta frecuente en el servicio de urgencias y constituye un reto para el personal de salud por el número de condiciones amenazantes de la vida que se acompañan de este signo clínico. La toxicidad en el sistema nervioso central es una de las manifestaciones más comunes en el paciente intoxicado por cuanto se han encontrado signos y síntomas hasta en un 30%. Este porcentaje es considerable en personas en edad laboral pues la morbilidad derivada de un retraso en la atención y las lesiones secundarias, implican repercusiones sociales. Según su acción en el sistema nervioso central los xenobióticos se dividen en dos grupos: estimulantes que causan agitación, delirium o convulsiones y depresores que provocan grados variables de alteración en el estado de conciencia, desde la somnolencia hasta el coma. El propósito de este artículo es revisar las sustancias depresoras, con enfoque en el coma de origen tóxico. Además de la fisiopatología y los principales tóxicos implicados, se plantea, con base en la evidencia, la mejor aproximación a esta clase de pacientes con el objeto de favorecer el uso razonable de los recursos disponibles y evitar complicaciones y disminuir costos al sistema de salud. ABSTRACTDepression of consciousness -coma, specifically -is a frequent complaint in the emergency room consultation and is a challenge for health personnel, considering the number of life-threatening conditions that may be associated with this clinical sign. Toxicity in the central nervous system is one of the most common manifestations in the poisoned patient, where signs and symptoms have been found in up to 30%. This percentage is significant in working-age individuals as morbidity resulting from a delay in care and secondary lesions involves a social impact. According to their effect on the central nervous system, xenobiotics can be divided into two groups: stimulants that cause states of agitation, delirium or convulsions, and depressants that cause varying degrees of alteration in the state of consciousness, anywhere from drowsiness to coma. The purpose of this article was to review depressants, focusing on coma induced by intoxication. In addition to reviewing the pathophysiology and the main toxic substances involved, we consider the best approach to this class of patients based on the evidence, in order to favor the reasonable use of resources available, while avoiding complications and reducing health system costs.
Se estima que aproximadamente setenta millones de personas viven con epilepsia, de las cuales 80 % viven en países de ingreso bajo o medio como Colombia. Las personas con epilepsia viven con necesidades insatisfechas. Esta enfermedad, a pesar de ser prevalente, no se ve reflejada en las prioridades de los servicios de salud. Por lo anterior, existen importantes retos en el tratamiento integral de esta enfermedad. La epilepsia, enfermedad estigmatizada, más allá de los gastos farmacológicos tiene un impacto psicológico y social que resulta en un espectro de discapacidad muy variado, el cual cursa con dolencias notables como la depresión y la ansiedad. Asimismo, las personas con epilepsia tienen una menor tasa de empleo que las personas sin epilepsia, incluso ocho años después de haber comenzado la terapia farmacológica. El estado libre de crisis, uno de los objetivos principales en la terapia de epilepsia, es difícil de alcanzar en algunos pacientes y, si bien la cantidad de medicamentos anticonvulsivantes ha aumentado notablemente desde 1990, las crisis pueden ser generadas a partir de múltiples mecanismos, lo que se traduce en un reto a la hora de buscar blancos terapéuticos para los medicamentos ya que varias etiologías se pueden traducir clínicamente en tipos de crisis similares. Aunque la monoterapia sea el ideal del tratamiento en epilepsia, la prioridad es alcanzar una reducción en la frecuencia ictal o un estado libre de crisis, por lo que si no se alcanza con el primer medicamento, la opción de añadir un segundo medicamento es aceptada plenamente. Existe una gran preocupación alrededor del uso de los medicamentos genéricos, los copia o multifuente y los originales, ya que la concentración y la absorción son muy importantes para su eficacia y seguridad, y es conocido que no deben variar más del 5-10 % para evitar cambios del patrón ictal de los pacientes. Se ha considerado que el cambio a medicamentos genéricos o multifuente está asociado con cambios en perfil de seguridad y en el patrón ictal, además de un aumento del uso de los servicios de salud y una alta tasa de cambio a medicamentos de marca, lo que podría aumentar los costos en salud. Por todo lo anterior, se considera que una atención óptima a los pacientes con epilepsia hace referencia a un manejo interdisciplinario en el cual se garantice el acceso y el seguimiento, para lo cual se deben aceptar retos como el entrenamiento del personal en atención primaria y la continuidad del manejo para evitar cambios en la biodisponibilidad, el perfil de seguridad y la toxicidad de los medicamentos.
El fenómeno de la toxicidad hepática inducida por medicamentos cobró relevancia hace algunos años con el estudio de las reacciones adversas a medicamentos. El daño producido en el hígado por un xenobiótico que altera su función es lo que se conoce como toxicidad hepática. La importancia de reconocer y diagnosticar la toxicidad hepática por medicamentos estriba en su gravedad potencial; no en vano es la causa más frecuente por la que la industria farmacéutica retira medicamentos. La tuberculosis es una pandemia que afecta a gran parte de la población mundial y junto con el VIH es una enfermedad cada vez más frecuente en Colombia. Esta enfermedad se puede considerar como una situación especial porque para su tratamiento es preciso suministrar, por largos períodos, medicamentos con potencial tóxico para el hígado. El objetivo de este artículo es revisar algunos aspectos relacionados con la toxicidad hepática secundaria a medicamentos antituberculosos, tales como: epidemiología, factores de riesgo, mecanismos de toxicidad, manifestaciones clínicas, diagnóstico, tratamiento y seguimiento.
