Since the introduction of contrast transesophageal echocardiography (cTEE) and contrast transcranial Doppler sonography (cTCD) the incidence of patent foramen ovale (PFO) has increased, especially in young patients with stroke of unknown etiology. A hundred patients with cerebral ischemia (60 with otherwise unidentified origin and 40 with different sources of cerebral ischemia) were prospectively examined by both methods. cTCD was used in all patients for the detection of microbubbles (< 8 µm in diameter) monitored in both middle cerebral arteries after intravenous administration. cTEE studies were made in 65 patients. Both methods agreed in 55/65 (85%); a right-to-left shunt was observed in 21/65 (32%), negative results in 34/65 (52%). Discordance occurred in 10/65 (16 %); in 8/10, only cTCD revealed positive results probably due to small pulmonary arteriovenous shunts which were not visualized by cTEE. In total, a PFO was diagnosed in 41/100; in patients without source of cerebral ischemia, 33/60 (55%) had evidence of a PFO; in 8/40 (20%), the results contributed to the detection of multiple possible sources of cerebral ischemia.
The contribution of computerized impedance cardiography in monitoring and differentiating cardiovascular responses to pharmacologic stress after the administration of dipyridamole (group 1, n = 24) or dobutamine (group 2, n = 26) was investigated during stress echocardiography. Heart rate, stroke volume index, cardiac index and systemic vascular resistance index were evaluated continuously with an automated, computerized, signal-averaged impedance cardiography system. Dipyridamole had little average effect on heart rate, stroke volume index, and cardiac index. The responses were similar in patients with positive (n = 9) or negative (n = 15) stress echocardiography test results (as characterized by echocardiographic wall-motion abnormalities). Dobutamine induced a similar mean increase in heart rate in patients with negative (n = 13) or positive (n = 13) results on stress echocardiography. The mean increase in stroke volume index induced by dobutamine was greater in patients with negative stress echocardiography test results than in patients with stress-induced wall-motion abnormalities. This distinction was also seen in the cardiac index; the mean change in patients with negative stress echocardiography test results was larger than in patients with positive results. It is concluded that automated computerized impedance cardiography not only allows surveying and monitoring hemodynamic changes during pharmacologic stress echocardiography but also contributes to differentiation of pathologic stress responses.
Gegenstand dieser Arbeit sind zunächst Besonderheiten des Fettstoff-wechsels bei Leberinsuffízienz, vor allem bei Leberzirrhose. Danach werden Argumente für und gegen die parenterale Fettzufuhr bei Kranken mit mangelhafter Leberfunktion erörtert. Ein weiterer Abschnitt bringt Tagesprofile energieliefernder Substrate und ihrer Metaboliten, wobei es sich um Untersuchungen an Zirrhosepatienten während einer peripher-venösen Ernährung unter Einbeziehung von Fett handelt. Bei abschließenden Hinweisen auf noch ungelöste Probleme steht die Frage im Vordergrund, ob man von Fettemulsionen negative Auswirkungen auf den neuropsychiatrischen Zustand zu erwarten hat. Die referierten Angaben der Literatur werden durch eigene Befunde ergänzt. Postabsorptiv zeigt das Serum von Patienten mit Leberzirrhose bei überwiegend physiologischen Triglyzeridwerten erniedrigte Konzentrationen der VLDL-Triglyzeride und fakultativ des Cholesterins. Die Lipomikronen-Elimination ist normal oder gesteigert, letzteres besonders bei großem portosystemischen Shuntvolumen. Zwischen der Fettklärung und dem Triglyzeridgehalt der VLDL besteht eine negative Korrelation. Die gaschromatographisch bestimmten Spiegel der unveresterten Fettsäuren (UFS) waren in einem Kollektiv von 35 Zirrhosekranken erhöht (p ≤ 0,0001), sie erreichten im Schnitt den doppelten Referenzwert gesunder Vergleichspersonen; dabei entsprachen die Anteile der einzelnen UFS an der Gesamtmenge weitge-hend dem physiologischen Muster. Faktorenanalytisch wurde nachge-wiesen, daß der Fettsäurenspiegel bei geringer und »mittelschwerer« Leberinsuffízienz mit dem Serum-Bilirubin und der Prothrombinzeit nicht korreliert ist, stattdessen aber – offenbar in Abhängigkeit vom Ausmaß des Leberumgehungskreislaufs – zum Plasma-Ammoniak und der Serum-Glukose gegensinnig variiert. Schwere Leberzellschäden bedingen einen Exzeß der UFS. Die Fettsäurenoxydation deckt bei Zirrhosepatienten trotz eines häufigen Defizits an Karnitin einen wesentlich größeren Teil des postabsorptiven Energiebedarfs als bei Gesunden (69 vs 40%). Dafür ist unter anderem die Forcierung der Glukoneogenese verantwortlich. Zur Konzentration der Ketonkörper liegen inkongruente Daten vor; niedrig-normale Werte von /3-Hydroxybutyrat können am ehesten der veränderten Leberdurchblutung zugeschrieben werden. Die Argumentation für und gegen die Verwendung von Fett bei Leberinsuffízienz berücksichtigt bevorzugt die Kohlenhydrat-induzierte Zunahme der hepatischen Lipidsynthese und die damit verbun-dene Leber-Steatose; hier wirkt Fett protektiv. Andere Leberschädi-gungen bei ausschließlich parenteraler Ernährung werden bezüglich ihrer Inzidenz, ihrer Phänomene, ihrer großenteils noch hypotheti-schen Mechanismen und präventiven Maßnahmen ebenfalls bespro-chen; sie sind durch Fettemulsionen nicht vermeidbar, diesen Emul-sionen aber auch nicht anzulasten. The paper gives a brief review of the existing literature concerning the nocturnal myoclonus syndrome (NMS). The clinical symptomatology, criteria for differential diagnosis and the relation to the restless legs syndrome (RLS) are discussed. Recently we investigated central dopamine receptor density with 123I-labeled 3'-iodo-6-methoxybenzamide (IBZM) (a highly selective CNS D2 dopamine receptor ligand) and single photon emission tomography (SPECT) in patients with NMS and found a reduced density of dopamine D2-receptors in the striatal structures, indicating a dopaminergic dysfunction in NMS and RLS. We present a report concerning a 58-year old female with NMS-associated insomnia and present IBZM SPECTs and hypnograms before and after a 3-month treatment with L-dopa and discuss the results with regard to pathophysiological theories.
