Ultrasound angiography represents a valuable additional imaging technique to colour-coded duplex sonography, providing further information, especially with regard to the border of plaques and stenoses.
Zusammenfassung
Ziel: Die Langzeitergebnisse der Varizenoperation zur Behandlung eines Ulcus cruris venosum sollten überprüft werden. Methode: Bei 63 Patienten mit vor fünf Jahren durch Operation einer Varikose der V. saphena magna behandeltem venösen Ulkus wurde der Langzeitheilungserfolg überprüft. Ergebnis: Von 63 unteren Extremitäten war in 49 Fällen innerhalb von sechs Monaten nach der Operation eine dauerhafte Abheilung des Ulkus eingetreten. Bei vier weiteren Patienten erfolgte die Heilung innerhalb von eineinhalb Jahren nach der Varizenoperation. Eine Langzeitheilung des venösen Ulkus wurde bei 84% der Patienten erreicht. Hierbei handelt es sich ausschließlich um Patienten mit einem Ulkus infolge einer Stammvarikosis. Bei 6% der Patienten (4 Beine) kam es zu rezidivierenden Ulzera, die z. Zt. unter Behandlung abgeheilt sind. Bei 6 Patienten (10%) blieb das Ulkus trotz zusätzlicher konservativer Therapie dauerhaft nach fünf Jahren weiter bestehen. Diese Patienten haben zusätzlich zur Varikose schwerwiegende Begleiterkrankungen (malignes Geschehen, PTS, AVK, Diabetes, ausgeprägte Lymphödeme etc.). Schlußfolgerung: Bei Patienten mit einer Stammvarikose und venösem Ulkus führt die Varizenoperation zu einem sehr guten dauerhaften Heilungserfolg. Eine zusätzliche schwerwiegende Begleiterkrankung beeinträchtigt die Prognose der Ulkusheilung erheblich.
ZusammenfassungBei 87 Patienten mit klinischem Verdacht auf eine Beinvenenthrombose wurden die Vv. tibialis posteriores und Vv. fibulares an 99 Beinen mit der Kompressionssonographie beurteilt und die Ergebnisse mit der Phlebographie verglichen. Phlebographisch wurden 39 Unterschenkelvenenthrombosen nachgewiesen. Sensitivität, Spezifität, positiver und negativer Vorhersagewert der Kompressionssonographie erreichten für die Unterschenkelvenen 89%, 98%, 93% bzw. 97%. Die Kompressionssonographie erlaubt eine hinreichend genaue und relativ schnelle Thrombosediagnostik im Unterschenkelbereich und ist zur differentialdiagnostischen Abklärung des Wadenschmerzes gut geeignet.
ZusammenfassungZiel und Methode: Bei 96 Patienten wurden wegen eines akuten, klinisch unklaren Wadenschmerzes eine Farbduplexsonographie und eine Sonographie der Wade durchgeführt. Ergebnisse: Folgende Ursachen des Wadenschmerzes wurden festgestellt: Wadenmuskelvenenthrombose bei 24% der Patienten, Thrombose der tiefen Unterschenkelvenen bei 15% der Patienten, Muskelfaserriss bei 13% der Patienten, Varikophlebitis bei 7% der Patienten, Hämatom bei 7% der Patienten, rupturierte Bakerzyste bei 5% der Patienten, Erguss im Kniegelenk bei 5% der Patienten, große Bakerzyste bei 3% der Patienten und Tumor bei 1% der Patienten. 20% aller Patienten hatten einen unauffälligen Ultraschallbefund der Wade. Schlussfolgerung: Eine zusätzlich zur Farbduplexsonographie durchgeführte Sonographie der Wade trägt wesentlich zur Abklärung eines unklaren Wadenschmerzes bei.
The aim of the study was to detect the frequency and the extension of thrombosis caused by central venous catheters by means of Doppler-duplex ultrasound. 36 patients were examined. The visualization rate of catheters in jugular (91%) and femoral (62%) veins was excellent. In contrast, catheters in the subclavian vein could be detected only in 33 per cent of all cases. As a result the overall patency rate of catheters was only 37 per cent after a period between 8 hours and 60 days (12.2 +/- 11.3 days) following the introduction. There was no significant relation between the type of catheter, the management with central venous catheters and the rate of thrombosis.
