In patients treated with lipid apheresis, a hyperhomocysteinemia can be frequently seen. The constellation of both elevated homocysteine and cystathionine levels points to the existence of tissue vitamin deficiencies, folate and vitamin B-6, which were improved by vitamin supplements. Because methylmalonic acid was mostly normal, a vitamin B-12 deficiency was not proven.

Ein von dem Gemeinsamen Bundesausschuss (G-BA) f?r Patienten mit schwerer LDL-Cholesterin- oder isolierter Lipoprotein(a)-Erh?hung und kardiovaskul?ren Ereignissen unter chronischer Lipoproteinapherese (LA) gefordertes Register wurde aufgebaut. Als Deutsches Lipoproteinaphereseregister (DLAR) ist es seit April 2012 im Routinebetrieb, in das alle Patienten, die regelm??ig mit einem Lipoproteinaphereseverfahren therapiert werden, erfasst werden sollen. Dieses Register wird von dem neutralen Tr?ger Wissenschaftliches Institut f?r Nephrologie (WiNe) vertreten. WiNe hat einen wissenschaftlichen Beirat eingerichtet und diesen beauftragt, die Daten- und Behandlungsqualit?t zu ?berwachen und in j?hrlichen Abst?nden eine Auswertung dieser Daten zu erstellen. Ziel des DLAR ist nicht nur die Erfassung der Lipidparameter vor und nach Apheresebehandlungen als Qualit?tssicherungskriterium, sondern auch z.?B. die prospektive Erfassung von Nebenwirkungen, kardiovaskul?ren Ereignissen bei Patienten vor und unter chronischer Lipoproteinapherese.
Targets for LDL-cholesterol (LDL-C) that should be aimed at have been defined by international societies. In high-risk patients they are set at 1.8 (70 mg/dL) and 1.4 mmol/L (55 mg/dL). We report our experience with respect to the use of statin and PCSK9 inhibitors in patients with familial hypercholesterolemia (FH). It was observed that in 37.5% of FH patients the target LDL-C level, corresponding to the risk category, was not achieved.
