Introdução: O uso de drogas acarreta danos aos familiares de usuários. O objetivo deste artigo foi analisar a percepção de codependência e dos impactos da drogadição e as estratégias de enfrentamento dos familiares de usuários de substâncias psicoativas. Método: Estudo qualitativo exploratório, com 16 familiares de dependentes químicos integrantes de um grupo da Cruz Azul do Brasil, entidade sem fins lucrativos, que realiza um trabalho social cristão diacrônico visando ajudar dependentes do álcool e de outras drogas, seus familiares e outras pessoas afetadas direta ou indiretamente. Uma entrevista semiestruturada foi realizada e analisada a partir da Análise de Conteúdo. Resultados: Houve predomínio de mulheres (93,8%), com ensino superior, e 50% dos cuidadores eram companheiros/as. As vivências emocionais dos familiares foram associadas à autorresponsabilidade, autocrítica e autossacrifício. Como estratégias de enfrentamento, foram identificados o grupo de apoio da Cruz Azul, a rede de apoio e a espiritualidade/religiosidade. Discussão: Ao vivenciarem a doença do usuário, arriscam-se a adoecerem juntos, seja por um processo de codependência, seja por doenças psicossomáticas e psicológicas. Conclusão: Intervenções incluindo os familiares dos usuários e com profissionais de equipe multidisciplinar seriam importantes para promoção da saúde dos envolvidos.
