Bu çalışmada hastanemizde laparoskopik obezite cerrahisi uygulanan hastaların retrospektif olarak değerlendirilmesi amaçlandı.
GEREÇ VE YÖNTEM:Şubat 2014 ve Aralık 2018 arasında tek hekim tarafından obezite cerrahisi uygulanan olguların dosyaları incelendi. Demografik özellikler, postoperatif komplikasyonlar, hastane kalış süresi, postoperatif taburculuk, mortalite kaydedildi.BULGULAR: 4 yıllık bir süreçte toplam 89 hasta opere edildi. Hastaların yaş ortalamaları 41±11,44, ortalama beden kitle indeksleri (BKİ) 46,26±6,74 idi. Hastaların 66'sı kadın, 23'ü erkekti ve bunların % 44,9'unda ek hastalık mevcuttu. Hastaların %9'u zor entubasyon (fiberoptik bronkoskop ile entübe edilen)olup %67,4'ü servise çıkarken, %32,6sı yoğun bakıma çıkarılmıştır. Yoğun bakım ihtiyacı BKI 50'nin üzerinde olan hastalarda anlamlı bir şekilde yüksekti (p=0,011). Hastaların %9'unde postoperatif komplikasyon görülürken, %2,2'sinde mortalite görülmüştür. Hastaların cerrahi süre ortalamaları 120,09±68,09 dakika olup hastane kalış süreleri 6,76±4,62 gün idi.
SONUÇ:Obezite beraberindeki hastalıklar nedeniyle kronik ilerleyici ve hayatı tehdit eden bir hastalıktır. Literatürle de uyumlu olarak hastaların çoğunluğunun kadın olması ve yaş ortalamaları düşük olmasına rağmen, %44,9'unda ek hastalık olması bunun göstergesidir. Laparoskopik sleeve gastrektomi uygulanacak hastaların perioperatif anestezi ve cerrahi uygulamalarının, hasta sonuçlarının ve komplikasyonların incelenmesinin başarı oranının artırılmasında önemli olacağı kanaatindeyiz.
Objectives
Although high procalcitonin (PCT) levels are associated with poor neurological outcomes and increased mortality rates in patients treated with targeted temperature management (TTM) in the postcardiac arrest (CA) period, there are limited data about the correlation between PCT levels and infection. The aim of our study was to assess the relationship of PCT levels in the first 48 hours with early period infections, late period neurological prognosis, and mortality in patients treated with TTM after CA.
Materials and methods
Serum PCT was measured on admission days 1 and 2. The early onset infection diagnosis before the seventh day in the intensive care unit (ICU) was made according to the criteria of infection centers for disease control and prevention. Mortality and neurologic outcomes were assessed 90 days after CA according to cerebral performance category (CPC) score.
Results
There was no statistically significant correlation between early period infection diagnosis and PCT levels at the time of admission, 24th, and 48th hours. Patients with poor neurologic outcomes on the 90th day had significantly high PCT levels at 24 (

= 0.004). There was no statistically significant correlation between admission PCT levels and neurological prognosis. While the correlation between mortality and PCT levels at 24 (

= 0.004) hours was significantly high, no statistically significant correlation was found between admission PCT levels and mortality.
Conclusion
In patients treated with TTM after CA, increased PCT levels were significantly correlated with poor neurologic outcomes and mortality. However, the elevated PCT levels were not significantly correlated with early period infections.

AimTo evaluate the inflammatory parameters and oxygenation in severe coronavirus disease‐19 patients who underwent extracorporeal cytokine adsorption (CA).MethodsPatients who underwent extracorporeal CA for cytokine storm were included in the study. The changes in oxygenation, laboratory parameters, and mortality rates were investigated.ResultsThirty‐six patients were included in the study. The hemoglobin, thrombocyte, and C‐reactive protein (CRP) decreased, and PaO2/FiO2 ratio increased (p < 0.001; p < 0.01; p < 0.001; p = 0.04, respectively). Twelve (33.3%) patients received a single session, 24 (66.6%) received 2 or more sessions. CRP and fibrinogen levels decreased, and PaO2/FIO2 ratio increased in the single session group (p = 0.04; p = 0.04; p = 0.01, respectively). In the multi‐session group, the hemoglobin, platelet, procalcitonin, and CRP levels decreased, and PaO2/FIO2 ratio increased (p < 0.01; p = 0.02; p = 0.02; p < 0.01; p = 0.01, respectively). Day 15, 30, and 90 mortality rates were 61.1%, 83.3%, and 88.9%.ConclusionCA with hemoperfusion reduced CRP and improved oxygenation; however, mortality rates were high.
Bu çalışmanın amacı travma sonucu gelişen ekstremite ve pelvis kırıkları nedeni ile ameliyathanemizde opere olan hastaların yatış sırasındaki hemoglobin düzeyi ve eritrosit transfüzyonu gereksinimi ile yoğun bakım ünitesinde (YBÜ) yatışı ve mortalite arasındaki ilişkinin araştırılmasıdır.
GEREÇ VE YÖNTEM:Travma sonucu gelişen ekstremite ve pelvis kırıkları nedeni ile 01.01.2018 -31.12.2018 tarihleri arasında ameliyathanemizde opere olan hastalar çalışmaya alındı. Hastaların medikal arşiv ve anestezi kayıtları retrospektif olarak incelenmiştir. Birinci aşamada anemisi olan ve olmayan hastalar ortalama geliş hemoglobin düzeyi, eritrosit transfüzyon ihtiyacı, hastanede yatış süresi, YBÜ'de yatış ihtiyacı, YBÜ'de yatış süresi, mortalite oranları açısından karşılaştırılmıştır. Çalışmanın ikinci aşamasında tüm hastalar eritrosit transfüzyonu alan ve almayan olarak iki gruba ayrılmış ve iki grup ortalama geliş hemoglobin düzeyi, hastane yatış süresi, YBÜ'de yatış ihtiyacı, YBÜ'de yatış süresi, 1. ay ve 3. ay mortalite oranları açısından karşılaştırılmıştır.
